From departmental document and/or MOA Examples: summary; courses taught; teaching philosophy; academic instruction; developing as a teacher; developing learning activities; lectures; assessments; by course name… Candidate responses-- spend an appropriate amount of time on less than desirable student ratings– be holistic Graphics can make information easier to read (examples ahead) RESPOND EXPLICITLY to action items recommended in prior reviews

Ongoing Research 1) Diabetes Management The first project is one for which I received an R21 grant from the National Institutes of Health, National Institute of Child Health and Human Development division (NICHD) to explore the feasibility, acceptability, and preliminary efficacy of an Internet-based contingency management intervention to improve blood glucose testing in teenagers diagnosed with Type 1 diabetes. Recruitment for this study took place at the Naomi Berrie Diabetes Center in New York City. I worked with post-doctoral fellows (name, name) of the National Development and Research Institutes and a statistician (name) of the Research Triangle Institutes on writing the manuscript from this study. We found that all teens enrolled in the study showed improvements in the number of glucose tests taken per day after receiving a very brief motivational interviewing session; however, participants in an incentive group showed significantly more increases in the number of glucose tests taken during treatment (especially younger participants), relative to a control group. Older participants, regardless of group assignment, were more likely to show a trajectory of maintaining increased blood glucose testing.
