2 Insurance Requirement:
SUNY requires that all students going abroad be “adequately insured” To be adequately insured, you must have: Direct-pay coverage to international providers 2. Comprehensive coverage (medical and mental health, for emergency and non-emergency situations) 3. Medical Evacuation and repatriation coverage

3 International Students
If you are an international student already covered by the GeoBlue health insurance, then you have already met the SUNY requirements for going abroad If you waived out of the GeoBlue insurance, you will have to show proof of coverage internationally, or you will be enrolled into the GeoBlue coverage

COST OF INSURANCE: $43.95 per month (through 8/14/17) (GeoBlue Study Abroad insurance plan includes medical evacuation and repatriation)

5 What does the plan cover?
SUNY Outbound Benefits What does the plan cover? 100% Coverage for illness and injury up to a maximum of $400,000 per illness or injury Medical Evacuation up to $1,000,000 Repatriation of remains up to $50,000 Accidental Death and Dismemberment benefit of up to $10,000 Maternity Care Outpatient prescription drugs including oral contraceptives and devices Inpatient and Outpatient Mental Health Care Bedside Visit Benefit of up to $5,000 Medical treatment received in the Home Country, if NOT covered by Other Plan up to $5,000

7 How Do I Get My ID Card? The Study Abroad office will provide a list of all participants to the Medical Insurance Office You will be automatically enrolled into the GeoBlue insurance Your student account will be billed for the insurance premium You will be billed for the shortest time period possible that still covers you for the duration of your trip If you’re staying abroad after your prescribed coverage dates and want to extend your insurance, just contact the SMI office to make arrangements

Adult primary Care providers will be automatically displayed. Look for the Network Provider Icon to identify Direct pay options. If you would like to search for other specialties or facility types, click SEE MORE.

13 Getting Care Outside of the US
Requesting Direct Pay If you already know the name of the doctor you will see, you can start the process by clicking on the Start a New Direct Pay Request button. There are 2 ways to begin the process. This is the first option

Requesting Direct Pay You can start the direct pay process from the provider’s profile. The second option is to go to locate a provider from the doctor search and then click the request direct pay button in the physician’s profile.

15 Now confirm if you already have an appointment or if you need assistance making an appointment. Remember that the instant GOP feature is only available if you already have an appointment scheduled and if you are suffering from an illness or injury. All other categories will follow the normal GOP process.

16 You will then receive confirmation of the request
You will then receive confirmation of the request. A copy of the GOP is available in the member hub and is sent to the provider. You can also access this GOP on the GeoBlue mobile App.

17 I Paid For Medical Care Abroad…Now What?
Save all receipts/statements from the visit to a medical provider Use the GeoBlue Students website to file an “E-Claim” Enter all pertinent information into E-Claim website Upload supporting documentation Submit to GeoBlue GeoBlue will reimburse covered expenses in a check sent to the address entered on the E-Claim website Step-by-step E-claim instructions will be provided in orientation sent out by SMI Office

18 I Have My Own Coverage Abroad…
If you have coverage that meets or exceeds SUNY’s minimum requirements, you can apply for a waiver Waiver packets on the SMI website: You fill in the wavier form, your insurance company complete the “Clarification of Benefits” form Return to SMI Office once complete Waiver requests must be received at least one week prior to your trip departure
