A titer is a blood test that measures your immunity to a disease. Get quantitative titers. Upload lab report.

8. MMR (Measles, Mumps, Rubella)
Titers showing immunity (for each).
2 vaccinations. OR

9. Varicella (Chickenpox)

History of Chickenpox.

11. Tdap (not DPT, Dtap, or Td)
1 vaccination.

You will not be registered for Spring 2018 until all requirements are met.
You do not have to use the Student Health Clinic, but it is recommended. Wait until classes start.
