7 Learning Outcomes Understand the importance of monitoring and reporting changes in a frail older adults health  Have an awareness of different assessment tools appropriate to use with frail older adults Recognise their role in the facilitation of self management techniques relevant to frail older adults and integrate these into practice Demonstrate an awareness of the role of key frailty experts in West Lothian including when and how to contact them

9 Case Study - Edith Edith is 84 years old and has a long history of extreme breathlessness due to COPD. Other medical conditions include Parkinson’s Disease and dementia. Recent months Chest infection Distress Anxiety Reduced mobility Risk of choking Recent falls Dry skin Incontinence Risk of malnutrition Over the last month her condition has deteriorated following a chest infection, she has frequent periods of distress and anxiety. Edith has become increasingly dependent on her carers requiring assistance to wash and dress, due to pain and reduced mobility. Her appetite is poor and she can cough and choke at times whilst eating and drinking. She lacks confidence to mobilise, having had a few falls in recent weeks. Her skin is dry and at risk of breakdown and she has episodes of urinary incontinence. BMI 17, MUST score 4
