Eligibility Criteria Fair Access to Care Services (FACS) Guidance 2003/2010 Eligibility (Current): Must be assessed at critical, substantial or higher moderate levels of risk and identify unmet needs. Not eligible (If needs are assessed as moderate or low): Assistance may still be provided with the provision of advice and information about other organisations and services that may be able to provide further assistance. What we Do Adult Care: Supports people with learning disabilities, physical disability and older adults to optimise and maintain their independence. We aim: To enable people (where possible) to remain part of their community whilst protecting those in vulnerable situations.

What we Do Prevention: information, advice, universal services, signposting, equipment (including Telecare) Prevention (AV) :Housing prevention triage (OT) and referral to Home Improvement Agency (HIA) or Futures Homescape Equipment and Major Adaptations Safeguarding: investigation and protection of vulnerable adults

Determine Eligibility: FACS Support planning: Focus on maximising social capital and achieving Outcomes/Goals Coordinate and Review Funded Services: Virtually managed Personal Budget (PB) or Direct payment (DP) Support Hospital Discharges/prevent admission
