Bromilow – Established in 1993, Bromilow is a vibrant Sunshine Coast company offering a range of services to assist people in and around their homes Bromilow works with clients throughout the region, from Caboolture to Pomona We specialise in providing aged care services and support to younger disabled people across the Sunshine Coast Bromilow's support options encourage clients to be as independent as possible. We focus on empowering clients to achieve their goals and attain the preferred level of control and enhanced quality over their lives

Bromilow Services Bromilow services include however are not limited to- Personal Care Domestic Services In Home Respite Care 24 Hour Care Assistive technologies- LETS program Social support Meal preparation and support Nursing Services Home and garden maintenance Planned activity groups- Gentle Movement Class, Banjo’s Morning Tea, Kawana Companions

Home Care Packages Funded by the Commonwealth Department of Human Services. The Home Care Packages Programme (HCP) is for people who have complex needs that can only be met by a coordinated package of care. HCP’s are to assist people to remain living at home independently and enhance choice and flexibility of individual care needs. There are four levels of HCP’s: Home Care Level 1: Supports people with basic care needs. Home Care Level 2: Supports people with low level care needs. Home Care Level 3: Supports people with intermediate care needs. Home Care Level 4: Supports people with high care needs

Home Care Packages - Eligibility
Access – No minimum age requirement, Target Group, Special Circumstances Assessment -– Health Professionals can make referrals for ACAT assessment through My Aged Care (MAC) Approval – Locate a Home Care Package, these are allocated to providers Bromilow hold - 62 HCP L2 and 25 HCP L4 HCP’s with Bromilow include Case Management. Bromilow’s case management involves - Individual assessment, goal orientated care planning, regular home visits, ongoing monitoring and informal reviews. All HCP services are coordinated with in an individual HCP budget.

Commonwealth Home Support Program (CHSP)
Funded by the Commonwealth Department of Human Services: Entry level short term, goal orientated program assisting frail people who are living in the community to maximise their independence CHSP can also assist carers to take a break from their usual caring role Eligibility – Older people over 65 and Aboriginal and Torres Strait over 50 Assessment - A home support assessment conducted by the Regional Assessment Service (RAS) is undertaken by a RAS Assessor once referral has been made through My Aged Care (MAC) The RAS Assessor then develops the client’s Support Plan / Comprehensive Support Plan

Commonwealth Home Support Program (CHSP)
Bromilow are funded to provide Domestic Assistance, Personal Care, Social Support and Flexible Respite Bromilow receive referrals for these services via the My Aged Care Service Provider portal CHSP services are reviewed on a 3 month basis Client’s individual needs are communicated to Bromilow via the Regional Assessment Service Support Plan accessed on the My Aged Care Service Provider Portal

Veterans Home Care (VHC)
VHC services can include Domestic assistance, Personal care, Safety related home and garden maintenance, In home respite care Eligible person are Veterans, War Widows and Widowers To receive these services, Gold or White Card holders need to receive a VHC assessment To arrange an assessment for services contact is to be made directly to VHC Bromilow receives referrals for the VHC program via the Veterans Home Care provider portal VHC determines the review period of each individual persons service plans

Department of Veterans Affairs Community Nursing (DVA)
The Department of Veteran’s Affairs Community Nursing Program provides home community nursing services for entitled persons to meet their assessed clinical and personal care needs Services may include medication management, wound care and hygiene assistance. Community nursing services are delivered by Registered Nurses, Enrolled Nurses and Nursing support staff. Eligible persons are Veterans, War Widows, and Widowers. Assessment for services comes from an appropriate health professional. This could be a GP, a treating doctor in hospital, a hospital discharge planner or a VHC Assessment Agency Bromilow receives referrals for this service directly Bromilow Registered Nurses complete an initial assessment and coordinate the commencement of services Bromilow Registered Nurses liaise with client’s appropriate health professionals as required No client fee

Queensland Community Care Services (QCCS)
Funded by the State Government - Previously known as Home and Community Care (HACC) and will be replaced by National Disability Insurance Scheme (NDIS). Provides basic support to people with a disability or condition that restricts their day to day living, whom are at risk of losing independence without assistance from Community Care Can include Domestic Assistance, Personal Care, Home and Garden Maintenance, Community Access, Social Support, and Transport, In Home or Community Based Respite, Nursing, Allied Health, and Counselling Services Most providers at capacity

Queensland Community Care Services (QCCS)
Eligible persons are under 65 Bromilow receives referrals for QCCS directly from client’s requiring services and are funded to provide: Social Support and Respite Domestic Services At capacity

Transitional Care Program (TCP)
Funded by State and Federal Governments: Provides short term support and assistance for older people post hospital discharge Aims to help improve an individual's capabilities and maintain independence for longer You can only access transitional care directly from a hospital stay The package of services accessed may include: - Physiotherapy - Occupational Therapy - Podiatry - Social worker - Nursing support for clinical needs such as wound or personal cares

AUTOMATIC PILL DISPENSER
Pill dose times are programmed into the APD and when due the carousel moves around to the one open compartment. The light flashes and an alarm beeps until the device is turned upside down to dispense the pills. Can be locked for extra safety.

The car bar and peace of mind pendant
Car Bar – A sturdy handle to hold onto when getting in/out of the car Peace of Mind Pendant Sends an SMS alert to pre-programmed mobile phone number/s and able to have a two way conversation through the pendant. Works anywhere there is mobile phone coverage.
