WHAT IS MUSIC AND MEMORY?
2008, Dan Cohen - to bring individualized music to residents of long-term care homes via iPods. Simple idea - if he were in a nursing home - favorite 60s music. Goal - residents access favorite music = easy, compact, and inexpensive. In 2008, a program was started by Dan Cohen to bring individualized music to residents of long-term care homes via iPods. His idea was simple - if he ever ended up in a nursing home, he wanted to be able to listen to his favorite 60s music. His goal was to assist residents in accessing their favorite music in an easy, compact, and inexpensive way. LeadingAge Oklahoma Music for the Ages

Music and Memory, Inc exploded – assist those with dementia, mental illness, or boredom. In Australia, Netherlands, Canada, and across the globe. Since that time, the MUSIC & MEMORY℠ program has exploded into an exemplary way for those with dementia, mental illness, or boredom to enjoy a happier, more fulfilling day-to-day life with music of their choice. WHAT IS THE LEADINGAGE OKLAHOMA MUSIC FOR THE AGES PROGRAM?
$353,000 grant from CMS/OSDH. Funds are from the Civil Penalty Revolving Fund. 100 Nursing homes. 2000 residents – 20 per nursing home. Other senior care providers may participate at a fee. The project is divided into two participation phases. Phase 2 will have 55 nursing homes. We are so excited to be accepting participants in Phase 2 of the LeadingAge Oklahoma Music for the Ages ~ A Music and MemorySM Program. The program is funded through CMS/ OK St. Dept. of Health with $353, grant. Funds are from the nursing home fines which are collected by the Department and deposited in the Civil Penalty Revolving Fund. The grant will fund 100 nursing homes at 20 residents per nursing home for a total of 2000 residents. The project is divided into 2 phases. Funding is provided by the Oklahoma Civil Monetary Revolving Fund (§ ), grant number , Office of Protective Health Services, Oklahoma State Department of Health (OSDH). OSDH is an equal opportunity employer.

WHO CAN PARTICIPATE? OK nursing homes interested in improving quality of life for those they serve – Medicare &/or Medicaid. Residents with Alzheimer’s and other forms of dementia. Other senior care providers for a small fee. Exceptions to Eligibility: Hospital-based long term care beds. Nursing homes with a primary focus on short stay residents. One laptop computer - separate eligibility. Intergenerational Volunteer Program. Obtain students/young adults for technology. One laptop computer for use solely with this Program (separate financial eligibility – will discuss this in a moment). LeadingAge Oklahoma will reach out to state, regional and local groups to develop and encourage participation in an Intergenerational Volunteer Program. This will assist in obtaining students/young adults to help with the implementation of the technology portion of the program. Volunteer brochure on website and sample Volunteer Code of Conduct. Scouting; HOSA; Music Teachers Assoc; Southwestern Oklahoma St U Music Therapy – sent to all students – several already contacted providers. LeadingAge Oklahoma Music for the Ages

Donation Campaign. Sample forms, polices, procedures and technical assistance. Assistance with a Donation Program to help obtain additional equipment to expand your program – even here at the conference! Asking community groups to assist – girl scouts, Alzheimer’s Assoc. Sample forms, policies & technical assistance. HANDOUT-Assessment Questionnaire Examples: Cleaning the equipment policy (sanitation). Assigning the iPods and reassigning them. WHAT IS THE FINANCIAL ELIGIBILITY TO RECEIVE THE LAPTOP COMPUTER?
Submit a letter – explain need. Based on financial, census or other circumstances. If you are interested in the laptop, just submit a letter with your agreement showing need. Need may be based on financial, census or other extenuating circumstances. There are now 25 (was 50) computers available for Phase 2 and LAOK will make the final decision on who will be awarded a computer. LeadingAge Oklahoma Music for the Ages

WHAT IS REQUIRED OF PARTICIPANTS?
Commitment to implement and maintain the Program. Sign an agreement. Individualized assessment and care plan. Personalized music playlist The agreement is available on our website – LeadingAgeOK.org The implementation will include an individualized assessment and add it to your current care plan for the use of an iPod Shuffle with a personalized music playlist as part of the care and treatment routine for these residents. WHAT IS REQUIRED OF PARTICIPANTS?
Minimum of five staff to complete the Certification & implement (also available if you have turnover) One from each category: Provide a minimum of five employees to complete the MUSIC &MEMORY℠ Certification and implement the program. These 5 staff consist of: Executive Leadership (i.e., Licensed Nursing Home Administrator or other organization-level executive with decision-making authority). Nursing Leadership (i.e., the Director of Nursing or their designee, Director of Staff Development). Activities department (i.e., Activities Director). One other staff of your choice. Lessons learned from other states: Highly encourage ALL to attend the orientation webinars and the certification webinars. TWO of the team members are to be identified as CO-LEADS. Executive leadership Nursing leadership Activities department One other staff of choice All staff need to help resident CNAs Two of these representatives must be project co-leads. WHAT IS REQUIRED OF PARTICIPANTS? Phase 2
Submit deposit fee of $ – refundable IF all requirements are met. Submit information for the research. Submit “Henry” stories. Replace lost/broken equipment. Monthly webinars -1st Friday A refundable deposit fee of $ (if all program requirements are met). Participants must submit information for the research component of this project. Primarily electronic surveys and also using quality measures we have access to. Just like the “Henry” story you watched earlier, you will collect and report on your own “Henry “ stories. We will provide a sample release form for families. The nursing home agrees to replace any equipment provided through the Program should it become damaged or lost. WHAT IS REQUIRED OF PARTICIPANTS?
Reassign equipment to new residents. Develop personalized playlist for each. Commitment range from: Phase 2: Implement June 2017 – June 2018. Agrees to reassign the equipment to a new resident should the original person no longer reside in the nursing home or declines further participation. This includes developing a personalized playlist for the new resident. The duration of this commitment will range from 23 to 14 months, depending on time of enrollment in the project (Phase 1, or 2). DATA COLLECTION Submit survey data throughout duration of Phase. Stop participation or fails to comply with requirements for three months, dropped from the project and required to return the equipment. The project deposit fee will NOT be refunded. Authorized representatives from participating nursing homes submit data (thru surveys) throughout the duration of participation. We will also use your Quality Indicator Reports. We are sensitive to your time – short electronic surveys. If at any point, a decision is made to stop participation in the research component, or if the nursing home fails to comply with participation requirements for three months, the nursing home will be dropped from the project and required to return the equipment. The project deposit fee will NOT be refunded. LeadingAge Oklahoma Music for the Ages

INTERGENERATIONAL Volunteer Program
Outreach to national, state and local organizations. Collaboration with high schools, colleges and professional groups. LeadingAge Oklahoma will reach out to national, state, and local groups to develop and encourage participation in an Intergenerational Volunteer Program. This will assist in obtaining students/young adults to help with the implementation of the technology portion of the program. The need for student volunteers to help with the technology is a key finding in the other state pilots has been the need for assistance with the technology piece of the program. HOSA = International student organization For Future Health Professionals. Goals To develop effective leadership qualities and skills. To develop the ability to communicate more effectively with people. To develop character. To develop responsible citizenship traits. To build self-confidence and pride in one's work. To make realistic career choices and seek successful employment in the health care field. To develop an understanding of the importance in interacting and cooperating with other students and organizations. To encourage individual and group achievement. Non-grant participants may participate in the grant educational seminars and webinars for an additional fee - $250.00 CEUs are available for an additional fee. Expand # of Residents Providers NOT participating in the grant may register and pay for the Music & Memory certification without financial support from LeadingAge Oklahoma. Non-grant participants may participate in the grant once they are certified on their own sponsored educational seminars at a fee of $ for two individuals (CEUs are available for an additional fee of $35.00).
