6. RFP Section 3 – General Terms & Conditions
The General Terms and Conditions contained in UTMB’s Rider 103 shall be made a part of and govern any Purchase Order, unilateral or bilateral Agreement, if any, resulting from this Request for Proposal.
Respondents may offer alternate provisions to the Terms & Conditions:
Must refer to the specific article or section concerned.
General exceptions are not acceptable.
Terms & Conditions determined to be unacceptable may result in disqualification of
the proposal.

7. RFP Section 4 – Scope of Work/Service Requirements
Employee Heath provides services to all UTMB employees to meet occupational health requirements. UTMB seeks a Respondent to provide the medical leadership and clinical services for UTMB employees required occupational health services of a Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).
Respondent shall write and maintain the standing orders that direct the daily activities of the Human Resources (HR) Employee Health Clinic.
Provide orders and parameters for health care worker immunization and tuberculosis (TB) testing to meet Texas Health and Safety code 224/Texas Senate Bill 7.
Provide guidance for management of blood borne pathogen exposures and reporting.
Set medical surveillance requirements for the research community inclusive of exposures to pathogens and biologicals.
Provide guidance to the Animal Research Program according to Institutional Animal Care and Use Committee (IACUC) requirements.
Provide injury and illness care according to Optimal Disability Guidelines (ODG) guidelines according to the UT System chosen network Injury Management Organization (IMO).

8. RFP Section 4 – Scope of Work/Service Requirement
Maintain all documentation in the electronic medical record Underwriters Laboratory/Occupational Health Management (UL/OHM) electronic medical record software.
Design and guide UTMB medical surveillance programs to protect the health and safety of UTMB employees from physical, chemical, cognitive, and reproductive hazards in the workplace.
Knowledge and expertise in Return to Work, Fit for Duty programs, Texas Workers Compensation and disease management.
Operational expertise in management of Family Medical Leave management.
Ability to conduct a medical job analysis based on the Department of Labor (DOL) physical requirements of work and a working understanding determination of the essentials functions of a job.
Conduct Department of Transportation (DOT) physicals for drivers of DOT regulated vehicles.
Provide occupational health leadership to UTMB health system, research community and correctional managed care (CMC) for a regionally dispersed employee population.

9. RFP Section 4 – Scope of Work/Service Requirment
Provide support upon activation of the UTMB biological containment unit for infectious diseases.
Advise for the safety and protection of UTMB employees in business continuity. Readiness to research novel medical responses prior to an event and keep a treatment plan ready for implementation in an emergent/urgent exposure.
Board Certification in Occupational medicine preferred.
Other occupational healthcare duties as requested and assigned.
Quarterly review of service evaluation conducted by the Director of Employee and Occupational Health based on customer feedback and contractual agreement.
Minimum Requirements
All MD and OD shall be licensed to practice medicine in the state of Texas and can produce licenses upon request.
Providers shall be available to work from the UTMB Employee Health Clinic, 400 Harborside Drive, Suite 125, Galveston, Texas A minimum of fifteen (15) hours a week between the hours of 7:30 AM to 4:30 PM and shall be available for occasional telephonic or consolation.
Respondent shall have five (5) years of experience in occupational healthcare provider services for to be considered for this RFP.
Respondents must have extensive experience working with academic medical centers or other larger (500+bed) tertiary care teaching hospitals.
Respondents shall provide documentation demonstrating their capability in meeting and performing all specifications of this RFP.

10. RFP Section 5 – Pricing Schedule
Pricing shall be submitted in excel format only
Attachment A – Pricing Schedule Respondents shall provide a cost proposal that details all costs and fees associated with the delivery of the services you have offered under your proposal. Include any and all one-time and reoccurring fees or charges. These costs shall be shown for the full contract term.
