After attending this presentation, we want you to understand: • How to get the most out of your StudentBlue insurance. • What’s covered when you visit the University Health Center (Medical, Dental, Pharmacy). • How much you pay if you go to off-campus healthcare facilities. Finally, have your questions answered by University Health Center insurance representatives!

3 About StudentBlue • Administered by Blue Cross and Blue Shield of Nebraska • Includes health, dental and prescription coverage for one low rate. • Enrollment and waivers are managed by Ascension Benefits & Insurance Solutions

4 Automatic Enrollment & Billing
• International students and graduate assistants are automatically enrolled. • Any other UNL student can voluntarily enroll on MyRed (enrolled for three or more credit hours). • UNL Student Accounts automatically bills the premium to students’ accounts.

5 Coverage Periods Spring/Summer semester • Jan. 1, 2017 to July 31, 2017

6 Dependent Coverage • Graduate Assistants can purchase dependent coverage at a reasonable rate. • Dependents must re-enroll for each coverage period.

7 How to Enroll Dependents
Got to nebraskablue.com/studentblue Click on “University of Nebraska – Lincoln” Click on “Enroll your Eligible Dependent Here”. This will take you to the Ascension website.

8 Spring/Summer Coverage Period
Premium Costs Premiums Spring/Summer Coverage Period Student $1,189.29 Spouse/Domestic Partner Each Child $678.85 Includes $10 admin fee per coverage period.

9 Graduate Assistant Rates
Spring/Summer Coverage Period Graduate Assistant Portion: 21 percent $249.76 UNL Portion: 79 percent $939.53

10 Health Benefit Summary
Benefits are maximized at the University Health Center • Unlimited policy year maximum • No deductible, coinsurance or copays • Insurance pays 100 percent of covered services, including: - University pre-enrollment health requirement immunizations and tests - Medical procedures - Annual exams - Visits with medical specialists — Referrals are not needed - Annual optometry exam and refraction (glasses and contacts NOT covered) - Laboratory tests - X-rays - Physical therapy - Counseling and psychological services

11 Health Benefit Summary
In-network provider outside of the University Health Center • $500 deductible • 80 percent/20 percent coinsurance • $2,200 maximum out-of-pocket expenses — includes deductible, coinsurance and copays

12 Prescription Benefit Summary
Benefits are maximized at the University Health Center Pharmacy • Unlimited policy year maximum • $5 copay for generic drugs • $30 copay for brand name drugs

13 Dental Benefit Summary
Benefits are maximized at the University Health Center Dental Office: • $1,000 policy year maximum • Two dental check-ups and cleanings are 100 percent covered • Other covered services – $50 deductible and coinsurance apply

14 Confirming Enrollment and Insurance Cards
Though international students and graduate assistants are automatically enrolled and charged for StudentBlue, • In order to expedite mailing of your insurance card, you must confirm enrollment in StudentBlue via MyRed each semester. • You MUST have your current local mailing address in MyRed. • If you do not confirm enrollment, your card will be mailed in January.

17 Dowload Your Card to Your Phone
Mynebraskablue.com • Log into your myBlue Account • Click the ID Card icon, then “View My ID Card” •Tap and hold the image to save to your device.
