Atrial tachycardia originating from the right superior pulmonary vein (RSPV) after sinus node crushing and SG stimulation. A: isochrone maps during baseline sinus rhythm before sinus node crushing. B: after sinus node crushing, SG stimulation induced pulmonary vein (PV) tachycardia (beat b). When tachycardia terminates, a junctional atrial rhythm ensued (beat c). Electrograms 1–4 are taken from sites adjacent to the early ectopic site during beat b of PV tachycardia. Arrows in A and B indicate earliest sites of activity during beats a–c.
