SCOPE OF THE WORKS MES E Ltd – This scheme involves concrete repairs to the columns and cross beams underneath the ramp. On the ramp itself, new waterproofing system, new re-surfacing, re-alignement of kerb with new gullies connection will be the main activities. Traffic Management will be required for this scheme. Traffic light system will be implemented Work will be carried out mostly during daytime hours, hrs to 17:00hrs Monday to Friday. Three week ends working will be taking place toward the end of the job for the waterproofing and re-surfacing activities SITE INDUCTION

WELFARE ARRANGEMENTS Toilets and welfare facilities are provided by the Client on their own premises. You have a responsibility to co-operate in keeping them clean and tidy. No area other than the facilities than the one provided by the Client must be used as a toilet. There is a canteen with tables and chairs for your breaks. There is hot and cold running water and soap. A microwave is provided for your use. A changing room is provided. Please do not leave any valuables in this room or the canteen as Concrete Repairs or MES E Ltd will not accept responsibility for any items lost or stolen. SITE INDUCTION

ACCIDENT AND EMERGENCY PROCEDURES
All accidents, however trivial, must be recorded in the Accident Book. (kept in the site office) The nearest Accident and Emergency Centre is at: New Cross Hospital Wolverhampton Road Heath Town WOLVERHAMPTON West Midlands WV10 0QP Telephone: IN AN EMERGENCY DIAL 999 Directions to the above A&E are displayed on the site notice board. FIRE EMERGENCY PROCEDURES (1)
OPTION A: IF YOU DISCOVER A FIRE Immediately operate the nearest alarm call point which is located at: ‘Fire Alarm Call Point location’ or an air horn in “CRL vehicles” If not being placed at undue risk and you are trained to do so, attack the fire with the appropriate appliances provided. Otherwise ‘Get out and stay out.’ OPTION B: ON HEARING THE FIRE ALARM The following emergency procedure should be activated: Advise the site management or call the Fire Brigade. DIAL 999 Give the operator your telephone number and ask for FIRE. When the Fire Brigade replies speak slowly and clearly and provide them with the answers to their questions. SITE INDUCTION

Give them the site address, which is displayed on the site notice board. DO NOT HANG UP THE PHONE UNTIL THE FIRE BRIGADE HAS REPEATED THE ADDRESS AND THEY HAVE TOLD YOU TO DO SO. Leave the site and report to the Assembly Point which is: Car Park REMEMBER: DO NOT STOP TO COLLECT PERSONAL BELONGINGS DO NOT RE-ENTER THE SITE GO DIRECTLY TO THE ASSEMBLY POINT SITE INDUCTION

SITE RULES These are listed on the site notice board and must be read as they will be enforced. Important rules include: Toilets and welfare facilities are provided by the client. You have a responsibility to co-operate in keeping these clean and tidy. No area other than the facilities provided must be used as a toilet. Appropriate PPE must be worn and maintained as directed. Safety helmets, footwear and hi vis clothing must be worn at all times whilst on site. Radios, or other electrical equipment are not permitted on site. Only 110 volt electrical equipment may be used. Smoking is not permitted in the site compound and welfare facilities. SITE INDUCTION

P.P.E. ON SITE The minimum P.P.E. for this site is:
Hard Hat Hi Vis vest or jacket Safety footwear You must wear specific PPE for your task as per your Method Statement and Risk Assessment. SITE INDUCTION

SCAFFOLD TOWER & MEWPS ACCESS
Scaffold tower will be the only safe method of work (while working at height). Visual inspection to be carried out before each use. If necessary, Cherry picker will be used for difficult access. Harness and lanyard to be used at all time. Weekly inspection need to be carried out by a competent person at regular intervals. SITE INDUCTION

PERMITS TO WORK You will need to use the following permits to work on this site:

HOUSEKEEPING The site must be kept clean and tidy at all times.
Skips are provided for waste and rubble. Make sure fire escapes and access routes are kept clear at all times. Do not allow waste to build up to form a hazard. SITE INDUCTION

OPEN DOOR POLICY If you have any questions or issues of a confidential nature that you wish to discuss, we operate an open door policy on this site, so feel free to discuss these with your foreman or the site manager. These will be dealt with in confidence. SITE INDUCTION
