Which plan works for you and your family?
Plan 1: the “Traditional” Office Visit Co Pays CostCare Co Pay $20 RX coverage for Generics prior to Deductible & Formulary & Brand coverage after $50 Deductible Employees can contribute up to $2,550 to Flex (use it or lose it rules apply)

Which plan works for you and your family?
Plan 2: the High Deductible Health Plan (HDHP) All medical services apply to Deductible. Once Deductible is met, 80% covered by MCPS and 20% by member RX applies to Medical Deductible. Once Deductible is met, co pays apply. No office visit co pays – you pay full (allowed amount) for services Health Savings Account (H.S.A.) compatible. You can contribute up to $3,400 Individual or $6,750 for Family coverage in Unused contributions roll over from year to year. Note: Spouses still are costing more in claims

Which rate do I use if I have Spouse Coverage
Primary: means your spouse has no other health insurance coverage. You and the MCPS Trust Fund pay the claims. Secondary: means your spouse does have other health insurance coverage This means that MCPS pays after your spouse’s primary plan pays.

Dental Coverage Dental coverage is automatically included when you enroll in the health plan If you do not wish to elect the dental plan, you must “Opt-Out” The Affordable Care Act (ACA) requires that we provide you with an opportunity to drop dental if you wish. Dropping dental will have no effect on your premium! It just means you will no longer have dental coverage.

Long Term Disability/Long Term Care
These benefits PROTECT YOU and YOUR FAMILY if you cannot work and/or take care of yourself. Rates vary according to the benefit you elect Spouses and family members are eligible to elect Long Term Care Contact Beth Glover at PayneWest if you’d like to review options and cost Over 65 or does your Spouse need coverage options? PayneWest Insurance can help evaluate the MCPS plans vs Individual and/or Medicare supplements. Contact PayneWest Insurance (see flier)

CostCare We will continue our relationship with CostCare by continuing in-school blood draws with follow-up consults. They will also do flu shots this fall. We will also continue the $20 co-insurance payment for CostCare treatments at their three locations for those on our traditional plan.

Broadway Pharmacy Broadway Pharmacy is no longer able to fill 90-day prescriptions for MCPS at the $10 co pay. Employees may still choose to fill their prescriptions at Broadway Pharmacy, but will pay 3 - $10 co pays ($10 a month) for a 90 day fill Valley Drug, Ridgeway Pharmacy and Cigna are available to fill 90 day for the $10 co pay.

REMEMBER….. Remember to show proof to Kacy if your spouse is secondary on our plan and has other coverage. Joint Processing Forms are online Thinking of retiring fill out a paper flex form If you have VSP you do not need to do anything Even if you are making no changes, login and check your information. If you have FLEX or do a HSA, you MUST login and re-enroll
