3 Operating Guidelines Use only FreeStyle Precision Pro Glucose Test Strips Do not use test strips beyond expiration date printed on the barcode label and outer box Test strips can be obtained from HRMC Main Lab Do not use wet, bent, scratched or damaged strips Use the test strip immediately after opening Do not use test strips if the foil packet has a puncture or tear Use the test strip only once and then discard Use only control solutions provided by the laboratory Do not scan a packet’s barcode and use a test strip from another packet Make sure that the meter and test strips are at room temperature. If the meter is moved to an area that is a different temperature, allow meter to reach new room temperature before testing Always wear gloves and follow hospital policy for PPE If glucose meter is not working properly please contact the laboratory for troubleshooting assistance

4 Control Solution Testing
Controls must be run every 24 hours to verify the performance of the glucose monitoring system Use only Medisense Glucose and Ketone Control Solutions When opening a new bottle, write the discard date on the bottle label Each bottle of control solution is stable for 90 days after opening or until the expiration date printed on the label, whichever comes first Do not use the control solutions after the expiration date Invert the control solution bottle several times to ensure thorough mixing before use. Invert and tap the capped control solution bottle to remove air bubbles from the tip of the bottle Wipe the control solution nozzle with a clean gauze or tissue before and after each test Do not scan one test strip foil packet’s barcode and use a test strip from another foil packet This may cause incorrect results to be generated Replace the correct cap on the bottle and tighten the cap immediately after each use

5 Perform a Control Test 1. Turn on Meter
7. Gently invert control 3-4 times. Apply small drop to test strip. 2. Select 2 – Control Test 8. Wait for meter to analyze result 3. Enter Operator ID 4. Scan solution lot number 9. Test result will appear as PASS or FAIL 5. Scan strip lot number 10. Repeat for High Level control solution 6. Insert test strip

6 Troubleshooting Control Failure
Eliminate any air bubbles in the control bottle’s tip Wipe away excess solution from the bottle tip prior to testing Scan or enter the barcode from the packet of the test strip used Confirm that control solutions and test strips have been stored at correct temperature Check that the bottles of control solutions have not been opened for more than 90 days and are within manufacturer’s expiration date. Use a new test strip for each test Use only FreeStyle Precision Pro Blood Glucose test strip is used Use only MediSense Glucose and Ketone Control Solutions. If quality control testing fails twice contact lab for further troubleshooting

7 Perform Patient Test 1. Turn on Meter
5. Confirm the Patient ID (if prompted) You may see one of the following four Screens 2. Select 1-Patient Test Re-enter the ID using the keypad 3. Enter Operator ID Press 2 to confirm the Patient ID or 1 to ReEnter the ID 4. Scan or enter Patient ID Enter the year of birth (eg. Enter “63” for 1963; then press the Enter button. Press Clear to re-enter the ID Press 2 to confirm the Patient ID or 1 to ReEnter the ID

10. Wait for meter to analyze result 6. Scan strip lot number 7. The Sample Type screen may appear. Press 1-Arterial/Capillary Press 2-Venous 11. Results are displayed 8. Insert test strip Important: If the blood glucose result appears to be inconsistent, there may be a problem with the test strip. Repeat the test with a new strip. 9. Apply blood sample Sample accepted will appear and automatically start testing Important: If patient’s blood sugar is <50 or >400; repeat immediately at bedside, If blood sugar is still <50 or >400 verify blood sugar with STAT Venous Glucose

Capillary Blood Sample Neonatal Capillary Blood Heel Stick Sample Use only fresh whole blood samples Make sure the sampling site is clean and dry before lancing Collect the capillary blood using an auto- disabling, single use lancing device Apply a drop of blood to the target area at the end of the test strip Use only fresh whole blood samples Make sure the site is clean and dry before lancing Collect the capillary blood using an auto-disabling, single use lancing device Apply a drop of blood to the target area at the end of the test strip Alternately, the heel-to-strip method can be used: Allow a hanging drop of blood to form from the heel and apply to the target area of the test strip. Results will not be impacted by gently touching the heel to the test strip.

10 Patient Testing Troubleshooting
Reasons Glucose Result may be Higher than Expected: Reasons Glucose may be Lower than Expected: Hematocrit is lower than the acceptable limit for the test strips, as indicated on the test strip instructions for use 15-65% Serum or plasma samples were used instead of whole blood Hematocrit is higher than the acceptable limit for the test strips, as indicated on the test strip instructions for use 15-65% Hyperglycemic-hypotension or shock (with or without ketosis) Severe dehydration, hypotension or shock Water or alcohol remaining on the puncture site Venous or arterial whole blood sample not tested within 30 minutes of collection

11 Patient Reporting Patient results will show on screen when testing is complete Results will automatically interface into Meditech Wirelessly Glucose meters can be docked for interface if wireless is down The following screen comes up when the meter is turned on. The icon on the bottom left indicates the wireless is enabled. The arrows to the right demonstrate data will be crossing. The boxes to the right of that demonstrate there is data to be transferred. The menu below will come up once the meter is turned off. The meter will attempt to send any unsent data prior to turning off. Options: Let meter send & then it will turn OFF. Or select #1 and the meter will go OFF. Without sending data. Unless there is a “hard dock” the meter will not send until the meter is turned back on.

What should be done if quality control testing fails? What should be done if the patient results is <50 or >400? What should be done if the strip lot number is expired? What should be done if the QC bottles are expired? What should be done if the glucometer is not working properly?
