5 PCMH Update

6 PCMH Update PCMH reports should be released today/around June 28th.
PCMH QA to schedule their onsite six month validation visits- visits will be scheduled between July 1 and September 30, 2016 (some practices have already received a call).  Please remember to complete your Arkansas Medicaid six month activities in the AHIN portal by June 30. 2015 Shared Savings payments likely available in October- they are looking at paying out approximately 80% this fall and the remaining amount in Spring 2017/after all reconciliation has been completed  December 31, 2016 Activities-- Activity J: Complete a short survey related to beneficiaries' ability to receive timely care, appointments and information from specialists, including Behavioral Health (BH) specialists (link to complete the survey is not included on the AHIN site yet) and Activity H: Childhood/Adult Vaccination Practice Strategy The 2017 AR Medicaid PCMH Program enrollment will be held August 15 – October 31, 2016.

10 Start with the Reporting tab, then go to the Meaningful Use Dashboard and start with the dashboard admin

11 Make sure to mark NQF0018, NQF0059, NQF0060, and NQF0024

12 Then make sure to save your settings

13 Greenway Meaningful Use reports will run overnight and be ready for view/print the next day

14 Go back to the reporting tab, and go to the Meaningful Use Dashboard- then go to Dashboard on the left and find results per provider

15 Greenway will only figure results per provider, so you will need to add each provider’s totals to get an amount for the whole clinic.

16 Here is an example of how it will look:

17 The results were entered into the portal for both Medicaid and Blue Cross Blue Shield

18 Well Child Check-Ups .

19 The resources we use to find patients that need Well Child Check-Ups are:

20 1. We go through the list of our Medicaid beneficiaries available through AHIN

21 2. We go through the list of our BCBS beneficiaries available through AHIN

22 3. We also use Web-IZ to find our patients who need immunizations and checking their records to see if a Well Child Check-Up is needed .

23 4. Starting in October 2016, we will be using a clinical alert through Greenway to help identify patients that need a Well Child Check-Up .

24 5. Amii reports- we use this report to find patients that need Well Child Check-ups and to clean up our caseload .

25 When it has been identified that our patient needs a check up, we contact them by phone or postcard/letter. .
