CCHN is Cayuga contact, with Human Services Coalition Covers all upstate NY, relying on rural health networks Provides support for CBOs to participate in DSRIP Aims to have 1,000 CBOs by spring DSRIP: Delivery System Reform Incentive Payment Program

12 Requirements for CBOs “Attend” 4-6 webinar session
Attend additional learning communities or informational sessions as needed/wanted by CBO Receive reimbursement for attendance and participation

13 CBO Program Eligible agencies
Allows CBOs to have a place at the table Provides funding for participation Creates format to provide input for our consumers Categories Eligible 501(c)3 Nonprofit Does NOT bill Medicaid Budget less than $5 million Works within fields of health and quality of life
