3 DAY OPPORTUNITY CENTRE
Introduction This day opportunity is being managed by Hospiva Care Ltd. Hospiva Care LTD is an Organization, established in 2007 with head office in Basildon. Hospiva Care is a recognized homecare agency with good reputation, currently providing domiciliary care with Essex County Council, NHS, day opportunity with Havering Council, And supported living with Baxley Council Hospiva Care promotes a true passion to deliver the best for our Service Users, by having a person centered approach to our service delivery. DAY OPPORTUNITY CENTRE An environment that we will make you feel like home. The day opportunity center provides a supervised day time home from home centre for adults whose independence have lessened due to ageing, disability or illness. The Centre is wheelchair accessible, with experience trained staff to assist.

4 Statement of Purpose This day opportunity aim to provider both centre based and community outreach support service, supporting the daily activities of service users from a physical, psychological, social, emotional and cultural perspective. These services will provide a high standard of care to meet the assessed needs of service users; to enable them exercise control and choice, be informed, stimulated, involved, make use of opportunities within their local community by offering an inclusive, dynamic and flexible environment in which a person is the centre (subject to risk assessment). Our services will be provided by experienced, highly trained staff maintaining privacy, dignity and respect to cultural needs at all times. Our service will also offer as much support as required whilst promoting independence to achieve good quality of life and maintain healthy wellbeing.

6 Service Description Medic2 portfolio of services includes domiciliary care and day opportunities. Supported Living Our service aim to deliver individualised care to every service users. We have the expertise in delivery of professional care services to clients in the community, both in their homes or at the day centre. Our Services include; Home care Service Day opportunity with variety of activities Outreach Service in service user home environment. Befriending Service Domestic Service for service users Respite or giving Carers a break Shower or bathing facility available at centre (if needed) Social stimulation and networking for service users Training for care support workers Re-enablement An excellent environment for socially inclusive service which is user led and person centred A tailor-made support plan to meet each individual’s need. Provision of Dementia care and cater to meet young adults with early onset dementia

7 Activities Here are a few of our Activities at the Centre:
We offer a caring, lovely and friendly atmosphere at the Centre, with a variety of activity opportunities to stimulate service users, as well as try and encourage people to chat and socialise. There will be a full time activity co-ordinator, along with regular social entertainer and speaker every month. Here are a few of our Activities at the Centre: Wii Crafts (arts, card making, pottery) Film Days Music / Reading Snooker BINGO Short Mat Bowls Scrabble Reminiscence Session Keep Fit Outings / Shopping / Walks Dancing Group (Ball dancing, line dancing) Any other interest would be facilitated for service users choice to be met.

8 Staffing Medic2 staffs are employed for their record experience, training, skills, dedication and professionalism. The Day Opportunity Centre will have policies and procedures that our staffs will be required to meet Our staffs meet the following checks before employment: NVQ 1 – 3 Experience Employment History Communication Skills Health Record CRB Checks References All our staffs receives Mandatory training as soon as they are employed. We have regular staff meeting and training refresher as required. Centre manager operate an “Open Door Policy”. Staffs adhere strictly to confidentiality and Data protection.

9 Meals This day opportunity centre provides nutritious home-made meals, cooked from fresh on the premises. Cultural Meal choice will be catered for, such as Halal meat, Curries, Caribbean, African delicacies etc Service user will have choice of hot & cold meals daily. They will be encouraged to choose from the daily menu any meal of their choice. Each choice will be sensitive to the service users dietary needs (if any). Meals includes 2 course and 2 sweets balance diet at affordable cost of £6.00 daily. Snacks, teas & coffee are available free throughout the day. Dietary needs are noted during assessment.

10 Transportation This day opportunity centre operate a good transportation system through the whole borough. We provide, as and when required pick-up and drop-off facilities with a trained driver and an appropriate escort. Other transport facilities includes: Private Taxi Service Hospiva Care Owned Vehicles

11 Open Times & Fees OPEN TIMES FEES
This day opportunity centre opens from 9am to 4pm FEES Day Opportunity Service with transportation £60 per day Day Opportunity Service without transport £45 per day

12 Monitoring Traditional method of monitoring the quality of the service will remain in place, for example regular satisfactory survey of service users and carers. We are continually working in partnership with placement authority to maintain excellent quality and standard of service. Complaint will be promptly dealt with and appropriate action taken immediately. Suggestion Box is available for your new ideas for the centre.

13 Policies Medic2 is committed to operate a fully Policies Centre, meaning all our policies are up-to-date according to regulatory requirements. We maintain and update all our operating policies such as: Health & Safety Protecting Vulnerable adults / Whistle blowing Complaints & Compliments Equality and Diversity Staffing & Recruitments Manual Handling Medication Food preparation & Hygiene Admission Policy – with regards to admission - A full risk assessment and support plan will be undertaken with all clients, in the presence of their carers or family ( where appropriate).
