Key feedback received From service users:
Need a safe space to go – somewhere where not being judged. Space needs to be appropriate – not too cramped or tightly packed; Provides a structure to the day, ‘reason to get up in the morning’; constructive things to do – sense of achievement; Benefits gained by individual accessing the services have a wider effect on family members and other people in the community; Need early intervention before problems escalate; Need to increase funding to mental health groups and peer support groups

Key feedback received From providers:
Currently there is potential for people to access services at too high a level; they can then become frustrated when refused help; There is a need to increase awareness of the help and support available at the preventative stage; Money needs to flow towards/ follow people with lower level needs and focus on prevention; There is a need to increase the types and capacity of community support available; Step-down from acute care needs to be improved; better range of options e.g. recovery

How feedback was incorporated into service design
Services have been aimed at provision within communities which can be preventative and provide enablement A commitment has been made to channel funding into community based preventative support and Support at Home where appropriate Awareness raising of help and support available is a key element within the Service Delivery Model Provision has been made within the Service Delivery Model to improve the range of support options available Outcomes required by commissioners have incorporated the need for people to have constructive things to do which lead to a sense of achievement Services will be easily accessible and focussed on individual needs and aspirations

Vision for the Service To enable people with emotional and mental health and wellbeing needs to aspire towards and achieve their own personal goals; To provide seamless, person centred support in communities to improve the mental health and wellbeing of people across each geographical service delivery area (Lot) of the county and help to prevent entry into more acute health and social care settings; To promote the principles of social inclusion, prevention, recovery/ enablement and independence

Outcomes for Individuals accessing the Service
Improving skills e.g. communication, relationship skills; Building confidence/ emotional resilience; Be enabled to return to/ commence employment via appropriate support methods (including activities such as volunteering/ work experience); Increasing independence/ live as independently as possible; Building community networks/ own support networks; Developing and maintaining family or other personal networks; Reduced feelings of isolation and loneliness/ reduction in depression; Developing the skills to make use of necessary facilities or services in the local community including public transport and recreational facilities or services; Developing the skills to run and maintain own home; Improved choice and control; Achieving economic wellbeing; Improving physical health/ developing a healthy lifestyle.

Elements of Service Delivery Model
Act as a single point of contact for individuals requiring community based support with their mental health and wellbeing; Awareness raising and promotion of the Service; Identification of need/ desired outcome in relation to each Individual accessing the service; Quarterly review of need in relation to each Individual accessing the service Facilitation of Self-Management and Peer Support

Service Delivery approach
Signposting and support to access suitable mainstream support options; Signposting and support to access a suitable specific support option / or direct support by the Service Provider; Referral to a specialist intervention (only after options 1 & 2 have been explored and ruled out); Identification of gaps in existing provision and referral to the Joint Board

Service Delivery funding
There is an overall envelope of funding for this community support option which will be split in the following way: Contract value for Geographical provision by area Central funding for additional grant resource to be agreed via the Joint Board and SCC Hand-out shows how this could work in practice

Geographical Service Delivery Areas - Lots

Partnership Working and Joint Board
Work in collaboration with each other where different providers have been awarded different geographical service delivery areas - ‘Lots’; Work closely with Somerset County Council in order to develop the support options over the life of the contract; Form an integral part of a Quarterly ‘Joint Board’ which will oversee the work of the service as a whole; Refer potential gaps in service provision to the Joint Board and propose solutions.

Inclusion of dementia and autism
Why have they been included? Expectations e.g. not turning people away if they are appropriate for the service / signposting to relevant options Similarities between the outcomes required by some individuals It is not expected for these conditions to be the focus of service delivery however this will be monitored and reviewed

Contract Opportunity This contract will apply to the geographic area covered by Somerset County Council. This tender will be divided into Lots as follows: Lot 1 – Sedgemoor and West Somerset Lot 2 – Mendip Lot 3 – South Somerset Lot 4 – Taunton Deane Anticipated Start Date: 01 October 2017 Contract Length: 2 years Extensions: 2 further periods of up to 12 months TUPE

Procurement Principles
Fair, open and transparent process; All types of organisations are invited to apply; SCC will conduct any procurement process in accordance with the Public Contracts Regulations Any tender would fall within Schedule 3 of the Regulations (Social and Other Specific Services) and would be run under the Light Touch Regime; Commercial and Procurement will facilitate the process; Deadlines are non-negotiable. 20

Framework Registration
Homecare Framework Can only register for Lots 4 – 6 Tier 1 Lots 1 – 3 are now closed LDOF Service Areas 1 – 3 require CQC registration Service Areas 4 and 5 are unregulated It is a two stage process to register for both frameworks: Stage 1: Selection Questionnaire or Pre-Qualification Questionnaire Stage 2: Quality Questions

Which Framework? Customers who require personal care only will be sourced through the Homecare Framework Customers who require support will be sourced through Service Area 5 – Daytime Support on LDOF However, we may also source packages through for other Service Areas on LDOF – subject to having the right range of mental health providers available

Call – Off Process Once providers have been approved onto the frameworks, they will be given the status of an ‘Approved Provider’. Approved Providers will receive an alerting them to a new care package when it is advertised on the Lot or Service Area they are registered for. Each package of care is sent out with some background on the individual and their outcomes. To respond Approved Providers will have to answer a few questions on how they will meet the outcomes for the individual and include a price.

What to do next? If you don’t have a Proactis Account…
Providers interested in registering for the ‘Regulated Homecare Framework’ should visit ProContract to view the advert, which will then give instructions of how to register on Proactis. Providers interested in registering for LDOF should with their company address, , phone number and request for an account to be set up

What to do next? If you do have a Proactis Account…
To register for the ‘Regulated Homecare Framework’ search for the advert on Proactis and Express and Interest. Stage 1 registration will then be sent to you. To register for ‘LDOF’ please to request to be sent the Stage 1 registration
