Care and Support at Home
One Service, 3 key elements: Short Term Intensive Reablement with 24 hour availability On-going reabling care and support with 24 hour availability Sitting Service Developmental contract to support the development of an outcome based approach

Overarching Aims Help people to become more independent and achieve their optimum wellbeing and full potential. Allow people to live well in the community where they have choice and control about how their needs are met. Help to prevent inappropriate hospital admissions. Be responsive and flexible to changing need. Help people with care and support needs to be involved in their local community. Work in partnership with people and providers to promote the highest sustainable quality of life. Meet important outcomes in the care and support plan. Have services that are based upon the social model of disability, social inclusion and enablement

INTENSIVE REABLEMENT

Vision Short term service 1 to 6 weeks
Ethos of helping people become as independent as possible by doing “with” and not “for”. Personalised and high quality care and support. Focused on clearly defined achievable outcomes that will help a person regain skills they have temporarily lost, or help them to do things in a different way. Reduce or minimise the need for longer term / on-going funded support.

Model The service will be split into 2 areas:
Area 1 – West Somerset, Sedgemoor and Taunton Area 2 – Mendip and South Somerset Opportunity for up to 2 providers in the county for the intensive reablement service There will be clear boundaries between service access to intensive reablement and the core service One off fee for reablement, incentive to move clients on Provider will support people into low cost / no cost community support or refer back to the ASC for longer term care 24 hour service availability

Free to service user for up to 6 weeks

Service User Evaluation
Joint responsibility of ASC and Provider Maximum threshold for evaluation is 28 days Evaluation will identify on-going needs when the service ends and eligibility for SCC funded care Will also identify provider role in support to local community resources

Transition Arrangements
Evaluation will trigger review for assessment for on-going service eligibility Personal Budget FAB referral Revised / new Care and Support plan Reabling provider risk assessment / assessment / care planning / work to date - shared with new provider for continuation of reabling ethos Ideally first visit is joint as a handover

Model 8 Zones, 2 to 3 providers per zone
Need for on-going support with reablement ethos Outcome focused Clustering staff: keeping local / working collaboratively Hourly rate per zone Provider will support people into low cost / no cost community support or refer back to the ASC for longer term care 24 hour service availability

SITTING SERVICE

Sitting Service Commissioned in blocks of care and support between 2 and 12 hours Planned or emergency provision To enable a carer to take a break from their caring role To enable service user and provider to maximise their time together in or outside of the home May be booked as a regular provision by the user or as a one off. Opportunity for wider range of provision when personal care needs are not required Ideally consistency of worker and flexible provision Ethos of creating sustainability by introduction of volunteers.

Overall….. Tender will be divided into 10 lots – bidders can tender for any number of lots Potential for partnership working/subcontracting – bids accepted from consortiums. We will set the pricing range for each service element and bidders will be asked to provide a price within that range

Good assessment and outcome planning Key partnerships for The sharing of skills between professionals and support staff Close and joint working with Hospital Integration Service, Community Teams and the Equipment assessment centre and Occupational Therapists Development of locality forums
