1 ICD Logger Quick Start Manual

2 1. Log in with your VCOM email address and temporary password that you were sent

3 2. First time you log in and each time you change rotations or locations, you should adjust your account settings. After each clinical period, please update your Rotation/Period Type and Location

4 3. From the ICD Logger welcome page you can navigate everywhere

5 4. Diagnostic, procedure and medications are entered on the Log page
Click on these three buttons to enter diagnostic codes, procedure codes or drugs You have 4 ways to make entries Your current rotation type and location Code entry Environment Patient ID entry Patient demographics Free text notes, no PII Submit

6 5. For each patient with multiple codes, enter and submit each code separately.
After submitting your first code for a new patient, enter additional codes using “Select Existing” on the Log page, which will open this window so you can pick from the list of your recent patients Once you have selected an existing Patient Tracking ID from the list, it will automatically fill in the demographics on the Log page. Enter the additional diagnosis, procedure or medication Repeat entering and submitting one code for a given Patient Tracking ID until you have finished with that patient using the “Save + Same Patient” button. To start making entries for another patient, use the “Save + Clear Form” button.

7 Continue making entries using the Log page… …and then you can review and edit your entries using the Dashboard page

11 ICD Logger hints Diagnostic entries are best done using keyword searches Procedure entries are best done using drop down menus Drug entries are best done using keyword searches, with doses in the notes The Top 10 entries for each rotation/period are provided, and you may find an appropriate code, for which you can then increase the specificity using the drop- down menus. Free text notes can be used as a memory aid (but do not enter any detail which may personally identify a patient) Other things Preferred that you do not use “unspecified” codes (colored orange) ICD defines a “Medical examinations” as diagnoses, so your initial exam selection can be found there. Another way of selecting the best code is to search the web for “ICD10 code for xxxx”
