If the LEA is applying for Title I, Part D, Subpart 2, the written agreement with the facility should have a recent date (within 2 years). Part 2: Program Evaluation and Assessment of Needs section is divided into three sections: Student Academic Performance, Staff Training, and Other. One activity must be indicated in each of the three sections. Part 3: Planned Activities section: Questions 1‐3, and 5 must be completed; For Questions 4a‐4f, check all that apply.
