2: Nationality form and consent form (this is one form) completed for group passport by Friday 21st July
Children must be British nationals
3: Apply for EHIC cards by Friday 8th September
4: Attach photo (standard size passport photo) to photocard and ask their child to sign it by Friday 21st July

5: Medical consent form by Friday 8th September
6: Further payment of £85 to be made by Monday 11th September
