MMR ( 2 doses) : K through 12th Hepatitis B ( 3 doses) : Pre-K through 12th Tdap Booster ( 1 dose) : 7th though 12th Varicella (2 Doses): Kindergarten and 1st 2nd, 3rd, 4th, 5th 6th 7th, 8th and 9th Varicella (1 Dose): Pre-K, 8th through 12th (Includes all retained students in appropriate grades 5 doses of DTP and 4 Or 5 doses of Polio HIB: from 2-59 months : doses vary depending on age Pneumococcal Conjugate – required for ages 0 –59 months

8 Immunization Exemptions
TME: Temporary Medical Exemptions are for students who are in the process of completing their age appropriate immunizations. Remember there is a two year lock of TME’s! That means that the time given to update a student’s immunization status may not exceed 2 years. Permanent Medical Exemption: Part C of the 680 Florida Immunization certificate. ASPEN : Found in the employee portal Click on Health resources link Click on Globe icon title health training resources Look for top tab titled Aspen Hearing/Vision Screening and click on it Select tutorial that pertains to your role in ASPEN ( Who uses Aspen ? District vision and hearing screeners, speech and language pathologists, school site follow up staff such as counselors)

11 Communicable Disease Surveillance and Reporting Procedures
Common Childhood Diseases found in the M-DCPS Facilitator Manual Must Report Vaccine related illness outbreak such as mumps, measles, and pertusis ( need medical provider documentation ) Must Report illnesses such as Tuberculosis, and Bacterial Meningitis Outbreak / Contact Investigation for 2 or more of the same illness at the same school site. Integration with office of Disease Control at the Dept. of Health Briefing # gives exact instructions and the template for reporting disease outbreaks.

13 When a parent/guardian reports an illness it is important to get supporting medical provider documentation to confirm the illness. If the parent / guardian reports that the student is ill in the hospital please get the hospital name, day of admission and last day of school attendance. This information must be placed on a communicable disease template scanned and ed to and along with the medical provider document if there is one. For more information please refer to briefing 19571

15 Chronic Disease Management
Includes parents/guardians filling out specific action plans and medication authorizations for metabolic conditions including: When the documentation has been returned to the your school site please your request for a school site training to on the specific condition, (Do not send the documentation). Please include a contact person and contact number so that the school nurse from the health dept. can follow up. If you are at a Health Connect in Our Schools site, the nurse at your school will follow up. Asthma Diabetes Seizure Disorders Severe Allergies Cystic Fibrosis

16 Health Suite / Clinic Log
The Miami-Dade County Health Dept. has requested that a record be kept of all students who visit the school clinic. The clinic log should be shared with the Miami-Dade County Health Department’s school nurse when she visits the school.

18 Medication Administration
Medications must be counted and the count must be documented when brought in ! 2 staff members must be trained in medication at each school site ! Medication Administration 2 times per year
