5 SB 82 Triage Grant Services
Child and Youth Crisis Stabilization Teams- engagement and crisis services aimed at youth detained by DCFS with multiple placement disruptions within the last 12 months and who meet Specialty Mental Health criteria. Mobile Triage Teams- key populations include homeless, older adults and veterans Forensic Outreach Teams- transition services between the jail and the community Crisis Transition Specialist Teams – linkage services to clients served by mental health Urgent Care Centers

6 Tracking Impact of Triage Services
After receiving Child and Youth Crisis Stabilization Team services- Only 6% were psychiatrically hospitalized Only 3% were detailed in juvenile hall 56% were successfully linked to mental heath services

7 Mobile Triage Team Quality Indicator Linkage to Mental Health Services
+For Clients Provided with Mental Health Referral, totals based on number of clients who were seen more than once. ++For Clients Linked to Mental Health Services, totals based on number of clients who were provided a mental health referral. +++For Clients Linked to Mental Health Service for 60 Days, totals based on number of clients who were linked to a mental health service.
