When am I Eligible for Benefits?
Your benefits will be effective on October 1, 2017 provided you turn in all documents by September 29, 2017. Completed Benefits Enrollment Worksheet Supporting documentation for covered dependents must be attached Birth Certificate Marriage Certificate/Registered Domestic Partner Certificate Parent-Child Relationship Affidavit and supporting documents All paperwork must be received within 60 days from August 21st (October 19, 2017) If your paperwork is received after the 60 day deadline, your benefits will take effect the 1st of the month after 90 days from the date all completed paperwork is received by the Benefits Office.

Which Dependents May I Cover?
Legal Spouse Registered Domestic Partner with State of California Please note that charges for domestic partners’ benefits are subject to Federal taxes Children up to age 26 Adopted, step-children, natural born Dependents under age 26 in a Parent-Child Relationship Requires submission of specific documentation – please ask for more information if this could apply to you Disabled child over age 26 Must have been disabled prior to age 26 Medical certification and CalPERS approval required

How Much do Benefits Cost?
Some benefits are fully paid by the CSU, others have a shared cost Voluntary benefits, such as additional life insurance, are paid 100% by the employee Deductions for health insurance, HCRA, DCRA and retirement are on a pre-tax basis Deductions are withheld on a prospective basis Example – the check you receive for work performed in September (October 1 pay date) will cover your benefits for the month of October Due to processing time, if you miss your first benefits deductions, you may owe retroactive deductions

Choosing a Plan It’s very important to select your benefits options carefully, as they cannot be changed except during annual enrollment unless you have a qualified change in family status It’s important that you notify us within 60 days of a change in family status. These include: Birth/adoption of a child Marriage/Divorce Loss of other coverage Contact us if you have questions as to what is a change in status

Dental Insurance The CSU pays the full cost for dental insurance for you and your eligible family members Two plans to choose from: DeltaCare USA (Dental HMO Plan) May only use primary dental provider Must select a primary dentist, or one will be assigned to you No annual maximum California only Delta Dental PPO Use any dentist Rates better with PPO or Premier dentists $2000 per person annual maximum

VSP - Vision Service Plan Basic or Premier Plan
Enrollment for you and eligible dependents in the Basic plan is automatic and at no cost to you! Monthly cost to upgrade to the Premier plan is: Enroll directly with VSP for the Premier plan Use participating providers and no claim forms are needed If you use non-participating providers, you will need to file a claim with VSP and wait for reimbursement Computer glasses are available as a second pair of glasses for the employee only under the Basic plan Employee $4.33 Employee +1 $16.13 Employee + Family $30.52

Flex Cash! Do you have medical and/or dental insurance already through another non-CSU employer plan? If so, you can receive additional cash in your pay check each month: $ per month if both Medical and Dental are waived $ per month if only Medical is waived $12.00 per month if only Dental is waived Proof of other coverage due at time of enrollment Please note, Flex Cash is treated as taxable income

HCRA Health Care Reimbursement Account
Pre-tax account allows for reimbursement for eligible Medical, Dental and Vision expenses not covered by insurance and considered medically necessary. Examples of eligible expenses: Co-payments for medical/dental/vision Glasses/contact lenses/laser vision correction Orthodontia Expenses not eligible include: Cosmetic procedures Items merely beneficial to a person’s general health, such as nutritional supplements Over the counter medications, unless prescribed by a physician Maximum annual HCRA contribution is $2,600 annually. Minimum is $240 annually. You must re-enroll each year during Annual Benefits Enrollment!

DCRA Dependent Care Reimbursement Account
Pre-tax savings account up to $5000 annually ($ per month maximum/$20 per month minimum) to pay for day care expenses necessary for you to be gainfully employed Dependent child under age of 13 or a spouse unable to care for himself/herself If married, your spouse must also be working or looking for work Must re-enroll each year during Annual Benefits Enrollment

CSU Employer-Paid Life Insurance
$50,000 of basic term life insurance and basic accidental death and dismemberment insurance for you Begins automatically on the first of the month after your date of hire Additional voluntary coverage is available

Voluntary Life Insurance With Standard
Guarantee issue of up to $150,000 for employees (no medical history needed) if you apply within the first 60 days of employment Spouse guarantee issue is up to $50,000 Child(ren) up to age 26 guaranteed up to $20,000 You may apply for additional life insurance at any time, but after 60 days of employment a medical history statement is required Coverage may be accepted or denied based on medical history Mail enrollment forms directly to Standard They coordinate payroll deductions with the State Controller’s Office

CSU Paid Long Term Disability (LTD)
Automatic enrollment starting on your date of hire – no forms needed After 180 days of disability, the plan pays 66 2/3 percent of your salary, up to a maximum benefit of $10,000 per month Designed to supplement other sources of disability benefits such as IDL, Social Security, CalPERS retirement, etc.

CalPERS Retirement CalPERS = California Public Employees’ Retirement System Eligible employees are automatically enrolled in a defined benefit pension program starting on first day of employment Pre-tax deductions will be taken from your paycheck Different benefit formulas depending on entry date into CalPERS Must be at least 50 or 52 at time of retirement Percentage factor tied to age at retirement

CSU Retiree Health Insurance
Minimum 5 years or 10 years of CSU employment, depending upon CSU hire date Must be at least age 50 or 52 at time of retirement Must retire within 120 days of CSU separation date to be eligible for lifetime retiree health insurance Contribution rates currently similar to those of active employees Rates change each year during annual enrollment Medicare enrollees pay less

Voluntary Retirement Plans and Deferred Compensation Plan
Administered by CSU through Fidelity Administered by State of CA (CalHR) Through Nationwide 403 (b) 457 (b) 401 (k) Taxability Deferred tax on investment; variety of investment choices.
