5th Grade Classroom Rules

Behavior Reports Level 1 & 2 infractions in Code of Conduct
Expectations/Incentive: Each class will implement a weekly three strike system. three strikes = behavior report to be signed by parent & returned More severe infractions may lead to immediate behavior report. 1 behavior report = No PAW movie Level 3 & 4 infractions (or repeated Level 2) The teacher will fill out a student discipline report & the student will be sent down to the office. Parents will be notified. 3 behavior reports in a 9 week period = discipline form Anthony

Homework for completion 100% effort with all work shown Missing/Late Homework Depending on amount of assignments in each subject, there will be a cumulative homework grade each nine weeks When a student fails to complete an assignment, point (s) will be deducted from the homework grade Assignment must be completed for the next day, or recess will be lost Anthony

Newer Mathematics Curriculum
We have updated the curriculum in grades K-8. Instruction will focus on inquiry-based methodology. Problem solving will be emphasized in all units. All Pennsylvania Core Standards for Mathematics will be addressed. GO MATH! will be our new textbook series. Each student will receive “mini-books” for all units. Students can write in their consumable books. There are new online resources that can be used at home for additional practice and reinforcement. Aaron

Health Office at K-O-K Nurse is on site the entire time we are at camp. All meds must have written documentation/prescription from the Licensed Health Care Provider. Parent permission form must be completed for these meds. This form is in the packet or available on the North Allegheny Health Services web site. Cannot administer medicine without parent and physician signatures. Send only amount needed for camp. Combine all medications and permission forms in Ziploc bag with child’s name on bag. All medication must be in the original container.

Health Office at K-O-K When possible, please have all meds in to the health office at least two school days before leaving for camp. This will allow time to organize and call you if there are any questions. Will not take medicine with another child’s name on it. Acetaminophen (Tylenol) will be available for the students provided you indicated on the medical emergency form that we can administer it. We do not take student meds from school unless you contact the nurse beforehand. Any OTC medications (ex. Multivitamin and Melatonin) require physician orders. Medication needs to be submitted to the health office by Sept. 21 Aaron

iPad Distribution Nights will be held at NASH
Grade 5 - September 14th at 6:30 p.m. Grade 4 - September 19th at 6:30 p.m. Check your for: Information on our insurance coverage Please accept or decline insurance coverage prior to the Distribution Night. iPad User Agreement Bring the signed copy of the User Agreement to the Distribution Night. Information can be found here: If parents can not attend, make up dates will be held at the individual schools. Information on these make-up dates will be in the school’s main office.

More than one child receiving a device?
Attend the Distribution night (suggested that you attend the program for your oldest child). Bring all signed User Agreements (one per child). Be sure to accept or decline insurance coverage for each device that will be received. Parents will take home the device of their oldest child. The devices for younger children will be delivered to your child’s school the morning after their grade level Distribution Night. There will be a table at each Rollout Night for parent to drop off their forms.
