? Foundation Training Expert Professional Practice
Faculty Membership CPD revalidation Populate the GPhC site Populate the Faculty site Faculty eligibility after 2 years …. May be with different pieces of evidence VT3 Training at regular intervals Populate the GPhC site This is how we see things fitting together at the moment. The Faculty is undertaken after 2 years of practice... The EPP e portfolio will form part of the requirement for the Faculty. It will also meet the need for GPhC CPD records. It may be that an individual starts VT3 and at regular intervals transfers selected information into their Faculty e portfolio and the same or other information into their GPhC e portfolio. All 3 should complement each other. Takes ~2 years to complete

What does Foundation Training look like?
Feedback progress ~2 years of training Supported by Tutor, Locality Tutor/ Educational Supervisor, regional PEC Competency framework Resources ePortfolio - Portfolio then Final assessment. 4. Assessment Development of evidence from workplace Different learning styles Flexible The training, while specialist in nature, is designed to be flexible to allow this to be accommodated within the workplace. Participants will benefit from a structured training programme with support from Educational supervisors. The participants may take up to 18 months to complete this training but this may be completed in a shorter period of time depending on the individual candidate. The module is supported by a competency framework which guides the candidate in the collection of appropriate evidence to demonstrate their competency, and candidates are able to utilise an ePortfolio to both access the resources for the module have a repository for the gathered evidence. Candidates also benefit from a structured assessment process with regular in training reviews where candidates can share experience, assessment of the portfolio of evidence and final assessments to complete the module.

Title and aspect of the Element Sign off by tutor, can also comment This is the view of the framework within the eportfolio… Notice the ‘tools’ that there are to help … the ‘I’ button for more information, the sign off facility, the magnifying glass… There is also a ‘pencil’ with which your tutor can provide brief feedback… Only a few lines required to show progress and feedback…

Clinical care of patients
Clinical Pharmacy Care plans, case studies mini – CEX CbD Care planning webinar Medical gases Moving onto Clinical… Requirement for care plans and case studies and in some areas some people will be using mini – CEX and Case Based Discussions as part of their evidence. This involves being assessed on the ward…. The rest of the areas are using care plans and case studies… It’s just that some areas are piloting the new documentation. So… for the care plans…. these need to be clear as to what your contribution was and why… It’s what we can Inputs and outputs….with some key results to show your progress. So as an ACTION state ‘Amlodipine dose reviewed because..’. and as your OUTPUT describe the actual outcome…’Amlodipine dose reduced to… ‘ Refer to any investigations that are relevant e.g. BP and also if this in line with a Guideline state the GL. It is good practice to have summary of care/case on front page with reflections in short para…. The reflections should include learning points from sharing the care plan/case with your tutor. The reflections will demonstrate what you would do differently the next time based on the current experience. This will show progression in your clinical work. Remember to cross ref where possible and to cover medical gases within a care plan or case… need to mention gases and specifics … not just about cylinders… PO2 etc.

The Cost effective section has been dramatically reduced over the past few years………Note use of new templates so no Rxs in portfolios. Moving onto E and T… need training plan… reflections on the actual training experience and not the subject topic e.g. clinical topic…. Good practice to have at least one feedback form from a member of the audience……

Ensuring the quality and safety of patients
Need to highlight own local process and involvement in process, SEA , Audit , SOP [draft and action, i.e. link theory and their practice] NES website for information about audit and SEA, risk management webinar, patient safety webinar…. Ensure trainee understands what is required when submitting an SEA and also how to do an audit with criteria and standards.... Clinical governance… plenty of personal examples that demonstrate inclusion in various activities….. You need to complete and submit and SEA and an Audit… see NES website for details or the very front page of the e portfolio.

Element 8 - Medicines information
Processes for the searching of information, critical appraisal of the literature and delivery of a reply to a healthcare practitioner. Range of enquiries. Templates instead of original enquiries. Critical appraisal recordings on NES website + MCQs to help you… Need a mix of Level 1,2 and 3 enquiries Medical gases… mentioned before… webcast area on NES website… cross ref and include specific detail….. Medicines information…. Variety of enquiries. Don’t all need to be done in MI centre… ensure you remember references (books) and work systematically. Note new templates to be used.

Evidence Using the Evidence Booklet ………..
Review each piece in turn…. look at the structure… don’t worry about the content e.g. clinical content…. Take about 5 mins per piece and comment after each piece has been reviewed….. This may take about 1 hour….. Review each piece and have a discussion. Have you completed any of the post it note questions at this stage? Cross them off and get ready to answer any outstanding at the end of the session before you close….You should have the facilitator notes to help with the evidence….Next is assessment….. This should take about 15 mins.

Assessment Portfolio assessment
Portfolios assessed, trained assessors, using scoring sheet, remotely Evidence is sampled according to an assessment framework Max 5 small pieces for action Max 2 larger pieces for action >5 small pieces or >2 larger pieces then a bit more time and defer to next assessment Written feedback e mailed to trainee Your tutor will be assessing your evidence on regular basis and will be commenting on it and giving your feedback… the more formal part happens later…. Your portfolios will be assessed informally by your Locality tutor at the end of Jan for May final assessment and at the end of July for Nov final assessment…and then actually assessed by a panel. There is a structure and a framework is used…. If there are > 5 small pieces the portfolio will be referred to the next assessment date. A small piece if where we may wish further discussion and this would be detailed in the portfolio feedback that trainees receive very soon after assessment… It is provided by NES Pharmacy… A large piece of remedial action would be a rehash of a case…..Not good……………

Scenarios x 4 to validate evidence in your portfolio Assessment of : Problem solving skills Knowledge and understanding of pharmacy practice Breadth and depth of pharmacy practice Awareness of personal limitations, Systematic approach to practice Response to questioning Using principles of OSCEs …. One assessor for each scenario, 10mins per scenario, After portfolio assessment, trainee has been given feedback and they will be clear what they may need to come prepared to discuss at the final assessment. The final assessment is either in May or November. It starts with a presentation (from portfolio) and the presentation topic may have been suggested in the portfolio feedback-to help…. There are then 4 scenarios and we are looking at systematic practice?? Response to questioning?? Awareness of own limitations…. Nothing too scarey as trainee will have done heaps and be proficient!! This is then a discussion about their portfolio and any aspects that the panel want clarification on……………. Then close. About a week later they will have received their feedback and fingers crossed their certificate.

What does a Foundation Pharmacist look like ?
competent, practitioner through learning and reflection in the workplace confident practitioner who can communicate effectively with other members of the healthcare team patient – centred practitioner who have robust ‘core’ clinical skills and the ability to apply a variety of patient safety tools (SEA, Audit, PDSA) resourceful practitioner capable of problem solving and making decisions in stressful situations safe practitioner, aware of limitations, refer as required a resilient practitioner, developing early leadership behaviours and skills.. who is aware of the importance of the wider healthcare team

Conclusions The Pharmacy Vocational Training Scheme - revised -
service redesign in Scotland. Training scheme aims to develop pharmacists through learning by their everyday experiences - not by underpinning knowledge alone. Research informs that whilst the training is challenging, it is viewed by the majority of trainees as being a way of receiving a broad structured training and feedback from experienced senior pharmacists Which in turn, in their view, leads to increased confidence and a sense of improved quality of the pharmaceutical care that they are able to provide to patients. Have we covered all items raised in the post it notes? Here are some conclusions…. On the next slide are some tips from trainees who completed previously…

Advice from previous trainees…
Write it up as you do it. I felt my evidence could have been better if I had done this… Set yourself deadlines. I set a target month to complete each section. Get on with the work Don’t ignore it, it wont go away… Don’t leave loads to write up at one time.. You will forget the detail. To do lists for each section are important. So……….
