Discuss complaint verbally or submit grievance form Employee-20 days Determine grievance is warranted, complete grievance form if not completed, open internal or external investigation, notify parties & PQI, file documentation Immediate Supervisor-2 days Gather facts, interview parties involved and witnesses, review MHCO policy, discuss with supervisor, analyze, come to resolution, document, notify parties & PQI, file documentation Appeal using grievance appeal form Employee-10 days Determine grievance is warranted, open internal or external investigation, notify parties & PQI, file documentation Administrator-2 days Administrator-10 days Determine grievance is not warranted, close the grievance as unfounded, document, notify parties & PQI, file documentation Refer to Administrator, document, notify parties & PQI, file documentation
