When to Contact AD on Cell Phone
To inform about Medical Emergencies If there is an issue with scheduled transportation If an official or referee does not show up If a player or coach gets ejected from a contest If something has the potential to make its way to Dr. Keenan or the School Board.

Building Permits and Transportation
A building permit is required any time you need to use school facilities for meetings, practices, games, conditioning, open gyms. Jennifer Presley will set up all building permits for the High School. Dakota Berg will set up all building permits for the Middle School. It can take 7-14 days for a permit to get approved by the building principal, superintendent, and business office. Last minute permits are a case by case basis. Outside groups need to go through the Board Office for permits. Jennifer Presley will arrange Transportation at both HS and MS.

Finances and Fundraising
A Google Doc has been created for each Athletic Team at the HS and MS to track salaries, expenses, transportation, game reports, and fundraising money. All purchases and fundraising will go through the Athletic Department, not the Athletic Boosters unless a fundraiser involves gambling or alcohol. Per O.R.C.- Money in Athletic Booster Accounts can not be paid directly to a coach or to a vendor. It must be donated to the school, then used by the school to reimburse or pay a vendor.

Fundraising Continued
All fundraisers need to get approved by the AD first, then the Building Principal, Superintendent, and Treasurer. Before going to AD, fill out the District Fundraising Form. Fundraising Forms can be downloaded from the Athletics website. If Fundraiser involves alcohol or gambling, still go to AD first so he can be aware of a potential fundraiser with the Boosters.

Athletic Boosters President: Lani Stafford
Boosters will run concessions at all events, sell merchandise, and run 50/50 raffles at highly attended events. A percentage of sales will be distributed to the teams that assist with the above duties. It is an expectation that Winter and Spring Sports assist boosters during the Fall Season, Fall Sports and Spring Sports assist during the Winter, and Fall and Winter assist with Spring Sports. We have a program philosophy, so MS coaches will be expected to help out at HS events.

Athletic Boosters Continued
When working events: Coaches, players, and parents (if needed) Players can not be in a concession stand unless a coach is present. Football games: Check-in at 5:45pm and leave after concessions are cleaned up. A total of 8 players and at least 6 Coaches/Adults are needed. Collecting tickets at gate is also a Football Duty. August 25th: Girls and Boys Basketball October 20th: TBD September 1st: Track October 27th: TBD September 22nd: Baseball and Softball October 6th: Wrestling

Final Forms All athletes at MS and HS need to be registered on Final Forms, all forms need to be filled out by parent, then signed by parent and student. When Parent registers it sends them an link and athlete an link. Parent clicks link and creates a FF password, fills out/signs forms, then student clicks link, creates FF password for site, and signs forms. Parents need to register student with their mapleschools.com address. This is the only hiccup we have had so far.

Final Forms Continued All coaches need to click the link that has been sent to their address, create a password, and log-in to Final Forms. All coaches will be able to view all forms, including emergency medical forms, on a mobile device. AD will print off paper copies of EMR forms for you. View the coaches playbook on Maple Athletics site to see all you can do. Head Coaches need to: Click on Roster button, then assign team button. Coach identifies if kid is on VAR, JV, JV/VAR, 9th, or cut. Update individual info: Jersey number, Height, Weight… this is what will be sent to other teams and media.

Coaching Credentials All coaches, paid and volunteer, must possess a valid pupil activity permit in the State of Ohio. This is a State Law, not an option. Each coach has a profile on Final Forms and all credentials will be tracked on Final Forms. You will get reminders from Final Forms close to expiration dates. NEW CREDENTIAL NEEDED: Lindsay’s Law requires every coach to take a free course every year on Sudden Cardiac Arrest. The link to the free NFHSlearn course is on the Maple Athletics Webpage. We need paper copies of all certificates in Athletic Office so we can generate the Final Forms Database. , drop off, Snail Mail…

Coach Evaluations Waiting to see if OHSAA implements a mandatory evaluation form this year. If not, there will be a form created. It will be shared with you all beforehand. Will be Board Approved. Most likely will involve AD evaluating head coach, head coach evaluating all assistant coaches.

Security All keys and security codes will start with the AD
Do not prop doors open! We are a closed campus. If you open any type of lock, lock it right away. Don’t leave it hanging there unlocked. The following areas have separate alarms: Athletic Center, Football Locker Room/Weight Room, High School, Middle School. Coaches need to enter the following doors and enter their code to avoid setting off alarms: AC- enter using swipe card at door 2, Football Locker room- use swipe card and enter code at external locker room door, HS- Main Entrance by Mustang Statue and code, MS- Main Entrance. Code only needs entered if alarm is going off. Code out if you are last team out.

Supervision It is an expectation that your athletes are never left unsupervised. This includes after returning from an away game or after a home event. If kids are getting picked up, make sure they text/call their parents when you are 20 minutes from returning to the school. A paid coach must be on the bus when traveling. Do not transport students unless you get permission from the parent. Students must ride the bus to an event and home from an event. A parent may take only their student home from an away event if they clear it with you in advance and fill out the Parent Transportation Form that will be created and ed to coaches.

Social Media Encourage athletes and parents to follow the Maple Heights Athletics Facebook Page. (we average 10,000 views 24hrs after a post) Coaches are encouraged to set up Remind101 with parents and athletes for quick mass communications. Be careful who you give your cell phone number to. Be careful what social media platforms you use, who you are friends with, and what you post online.
