Request for Application (RFA)
Soliciting applications from Community-based and Faith-based organizations that will produce innovative strategies to help improve the health of local communities by incorporating Social Determinants of Health (SDOH) into implementation strategies.
Up to thirty-five (35) grants for a maximum amount of $10,000 will be awarded to Community-based or Faith-based organizations which are chartered in the State of TN as organizations or businesses (including but not limited to those designated as 501c3 organizations).
Anticipated Contract Period: July 1, 2017 – June 30, 2018
