Promote polices, programs and services designed to improve health and quality of life by preventing and controlling disproportionate burden of disease, injury and disability among disparities populations, particularly racial and ethnic minorities Serve as a leader throughout the state through partnering and engaging to recognize and address health disparities

5 2017-2018 Community-Based & Faith-Based Initiatives
Request for Application (RFA) Soliciting applications from Community-based and Faith-based organizations that will produce innovative strategies to help improve the health of local communities by incorporating Social Determinants of Health (SDOH) into implementation strategies. Up to thirty-five (35) grants for a maximum amount of $10,000 will be awarded to Community-based or Faith-based organizations which are chartered in the State of TN as organizations or businesses (including but not limited to those designated as 501c3 organizations). Anticipated Contract Period: July 1, 2017 – June 30, 2018
