Redesigned CHS Dollars Primary Care Opened Clinic in Longview, 2004 moved to current facility in 2011 Current Providers: 1 FTE Clinic Manager 1 FTE Medical Director .1 Physician 1.3 FTE Nurse Practitioner 1 FTE Registered Nurse 1 FTE Health Educator 1 FTE Community Health Representative .15 FTE Registered Dietician 2 FTE Tribal Assistors Currently recruiting for an additional 1 FTE Physician

7 Longview- Health Clinic
See all enrolled AI/AN, descendants, some family and employees with billable resource. 300 + Primary Care Visits per month. About 50% of patients from 150+ tribes. EHR has been fully implemented. (New enhancements implemented ongoing)

8 Longview- Mental Health
Began providing services in 2004 Provides individual and group therapy for youth and adults; provides family therapy and parenting groups. Psych ARNP provides medication management and therapy. Therapists work with local school districts to provide services for AI/AN youth and families. Clinicians use EHR CARF Accredited Current Providers: 1 FTE Program Manager 4 FTE Mental Health Counselors 1.4 FTE Psych ARNP

9 Longview- Chemical Dependency
Began providing services in 2004 Provides adult and youth outpatient alcohol/chemical dependency services WA State Certified CARF Accredited Currently, youth served are primarily non-Native. Healing of the Canoe (HOC) Prevention curriculum has been developed and is being implemented in (HOC is funded by a county grant) Counselors use EHR Current Positions: 1 FTE Program Manager 8 FTE CDPs

10 Longview- Vocational Rehabilitation
Began providing services in 2004 Provides Job Preparedness and Search Support for enrolled members of federally and state recognized tribes from, Clark, Cowlitz, and Lewis Counties. Must have a documented  disability that is a barrier to employment. (Physical, Mental, Learning, Substance, Etc.) Helps dozens of AI/AN annually to attain and maintain sustainable employment. CARF Accredited. Only Tribal 121 Program in the nation with this distinction. Funded through the Department of Education, Rehabilitation Services Administration.

We offer co-occurring behavioral health services 3 dual credentialed providers (Mental Health Counselor and Chemical Dependency Professional) Culture is incorporated into services

13 Seattle- Mental Health
Began providing services in 2010 Provides individual therapy for youth and adults; provides family therapy and parenting groups. Hired 1 FTE Psych ARNP in December 2015. Psych ARNP provides medication management and therapy. Therapists work with local school districts to provide services for AI/AN youth and families. Therapists conduct several hundred visits per/month Therapists use EHR CARF Accredited Current Providers: 10 FTE Mental Health Counselors 1 FTE Psych ARNP 1 FTE PHD Psych 4 FTE Associate Mental Health Counselors

14 Seattle- Chemical Dependency
Began providing services in 2012 Provides adult and youth outpatient alcohol/chemical dependency services WA State Certified CARF Accredited Working on MOU with School Districts in King County to provide services to AI/AN youth in coordination with current mental health program. Counselors use EHR Current Positions: 4 FTE CDPs

Began providing services in 2009 Provides individual and group therapy for youth and adults; provides family therapy and parenting groups. Psych ARNP provides medication management and therapy. Therapists work with local school districts to provide services for AI/AN youth and families. Clinicians use EHR CARF Accredited Current Providers: 2 FTE Mental Health Counselors .6 FTE Psych ARNP

17 Vancouver- Chemical Dependency
Began providing services in 2006 Provides adult and youth outpatient alcohol/chemical dependency services WA State Certified CARF Accredited Counselors use EHR Current Positions: 2 FTE CDPs

18 Vancouver- Advocacy services “Pathways to Healing” Program
Provides advocacy services for self-identified AI/AN who have been affected by sexual assault, domestic violence, dating violence or stalking. Serves Clark, Cowlitz, and South Lewis Counties. Services may include: legal advocacy, education, transitional housing, clothing, transportation, other financial aid, etc. Programs are funded through Washington State’s Office of Crime Victims Advocacy and the Department of Justice.

19 Toledo- Elders Program
Provides Congregate and Home Delivered Meals to AI/AN in Lewis and Cowlitz Counties. Provides Transportation, Caregiver Support, Cultural Activities, Trips and More. Funded through federal Agency on Aging.

20 Indian Child welfare Staff one ICW Social Worker to manage cases across the state. Case load ranges between at any given time. Funded by WA DSHS Contract. Kids Camp, 2010

21 Contract health services (CHS)
Began providing services in 2004 Serves Cowlitz Indian Tribal Members in the CHSDA. CHSDA includes: Clark, Cowlitz, Skamania, Wahkiakum, Columbia (OR), Lewis, Thurston, Pierce, King, Kittitas. Tribe has over 4000 members (About 2000 in CHSDA) Following WA counties have 300+ members: Clark, Cowlitz, Thurston, Pierce, King. CHS funds are used to provide specialty services such as optical and dental not available at a tribal clinic.
