2. PATIENT EDUCATION
Patient has received counseling in the proper use of warfarin
Complete the clinical intervention record in CIS

3. Pull up patient profile in CIS

4. Click on View and then Ad Hoc Charting

5. Click on Pharmacy

6. Click on Pharmacy Clinical Interventions

7. Click on Chart

8. All yellow boxes need to be completed

9. If choose Other, complete a reason why

11. Continue to scroll down

12. Complete the boxes and click on the check mark

13. Close the Ad Hoc Charting

14. A record of each intervention is found under Forms and the date and time it was done

15. ADVERSE DRUG EVENT
Patient is on Cipro 500mg po bid (9am/5pm) and FeSO4 325mg po daily (9am)
FeSO4 is rescheduled for 12 noon
Complete the clinical intervention record in CIS

16. Pull up patient profile in CIS

17. Click on View and then Ad Hoc Charting

18. Click on Pharmacy

19. Click on Pharmacy Clinical Interventions then click on Chart

20. Make appropriate selections

22. Complete the boxes and click on the check mark

23. Close the Ad Hoc Charting

24. A record of each intervention is found under Forms and the date and time it was done; double click on the record to see the completed documentation
