3 Overview Program Description: Work-Based Learning: Cooperative Education (WBL: COOP) is a program designed to give students supervised experience in a field of interest. Students must meet the minimum requirements for participation and be accepted into the program. Students must have an approved worksite continuously. Students will be released early from school to report to their job placement. Following enrollment in Cooperative Education, any student who have not retained an internship or apprentice for more than three weeks will be enrolled in another class for which they can receive credit. Course credit will be awarded to students per course for each 140 work (or internship/apprenticeship) hours at an approved training site. Completed employment forms are mandatory for students to participate in the Cooperative Education Program and receive credit for work off campus.

4 Overview Purpose and Goals: Work-Based Learning: Cooperative Education is an elective program that provides students with the opportunity to explore career development and employment. Students build on prior knowledge, interests, and skills that enhance preparation for future career opportunity and continuing education/training. Additional workplace skills are reinforced in ethics and social responsibility, personal finance, technical knowledge, leadership, and teamwork. The goal of the program is to lead to further education and employment. Cooperative education will help empower students to become competitive employees and productive citizens. in the Cooperative Education Program and receive credit for work off campus.

5 Prerequisites Prerequisites:
The student has had one (1) CT course credit The student has completed Career Preparedness Documentation that the student has achieved the content objectives of Career Preparedness.

6 Student Eligibility Student Eligibility The student is:
 Is at least 16 years of age.  Is classified as an 11th or 12th grader.  Is in Good Academic and Disciplinary Standing with the school  Is on track for graduation.  Good attendance  Has a clearly defined career objective.  Possesses the knowledge, skills, behavioral qualities, and abilities required for successful employment.  Employed in an Internship or Apprenticeship position

7 Internship and Apprenticeship
Apprenticeships are paid work experiences for eligible 11th and 12th grade students where experiences, hours worked, and wages earned are monitored and documented by the employer and the coordinator Internships are unpaid work experiences for eligible 11th and 12th grade students where experiences and hours worked are monitored and documented by a supervisor/mentor and the coordinator.

8 WBL: COOP Requirements
Required component of Cooperative Education 140-clock hours of instruction to earn one unit of credit per course Can be taken 4th (Semester) and 5th Blocks (Year-long) Please Note: 5th Period WBL: Coop is a year long class. Students will earn 1 Credit for WBL: COOP after successfully completing the course requirements while enrolled in Terms 1, 2, 3, and 4 at the end of the school year/May. (½ Credit will not be earned each semester).    

9 Reporting Class Attendance
Attendance is expected and will be checked daily, and absences and tardies will be recorded. This class will use the Canvas Learning Management System for attendance, announcements, etc.

Good Academic Standing in the course. Attendance: Maintain good attendance in the class. Maintain Good Disciplinary Standing. Always displaying Professionalism at school and work. Good Time Management: Managing your time wisely to complete ALL course assignments and remain in good standing at your place of employment.

13 WBL: COOP Interview Questionnaire
All students are required to complete Interview Questionnaire for the WBL: COOP Program and submit to the WBL: COOP Instructor. Please make sure that you type complete sentences to answer each question. All questions must be answered and typed to receive Credit.

14 WBL: COOP Application &
Teacher Recommendations All students are required to complete a application for the WBL: COOP Program and submit to the WBL: COOP Instructor. All students are required to submit 3 Teacher Recommendations from your former teachers and submit to the WBL: COOP Instructor. Student will complete required section of the form before submitting to your former teacher. All forms are required to be typed to receive Credit.

WBL: COOP Student Information Sheet All students are required to complete a student information sheet for the WBL: COOP Program and submit to the WBL: COOP Instructor. All forms are required to be typed to receive Credit. This document provides your contact information and other information for the course.

2. WBL: Training Agreement 3. WBL: Training Plan These forms are a requirement for the WBL: COOP Program and must be complete as instructed on each form. All forms are required si signatures as indicated in the directions. All forms are required to be typed to receive Credit.

17 WBL: COOP Policy Student acknowledges that the primary purpose of Work-Based Learning is educational and, therefore, agrees to abide by the Work-Based Learning (WBL) program policies and decisions of the Cooperative Education Teacher-Coordinator, including those regarding specific job placements Student acknowledges that the school, through the Cooperative Education Teacher-Coordinator, is acting as an intermediary between the training mentor and student and that the Cooperative Education Teacher-Coordinator has a legitimate right to know and a significant role in determining the outcome of any placement issues including, termination, scheduling, assignments, and all other aspects of student placement. Work-Based Learning students who fail to perform satisfactorily in all subject areas during any grading period and who fail to improve during the next grading period should be asked to resign from his/her placement. A student suspended from school should not be allowed to attend their WBL placement during the suspension. On the second offense he/she may be dropped from the Work-Based Learning program with a loss of all credit. A student must comply with the LEA attendance policy to participate in the

18 WBL: COOP Policy A student losing his/her WBL placement due to any action deemed unacceptable by the school and Cooperative Education Teacher-Coordinator will be dropped from the program with possible loss of all credit. A student whose WBL placement is terminated for any reason is to report to the Cooperative Education Teacher-Coordinator. Failure to do so may results in the student being dropped from the WBL program. A student not attending regular school classes, related study, and/or the Cooperative Education Seminar classes cannot work at the WBL placement on the day(s) he/she is absent. In case of absence, the student is required to call the Cooperative Education Teacher-Coordinator and his/her training mentor before class or working period. Personal business handled at the WBL placement is prohibited. Friends or family are not to visit the student at the WBL placement. A student is to be on time at school as well as the WBL placement. Parents should understand the student’s responsibility to the training WBL placement and not interfere with the performance of his/her duties. Business rules for dress and personal hygiene will be observed.

19 WBL: COOP Policy Since training is the primary objective, a student is expected to remain with the WBL placement to which he/she is assigned. Students may resign or change placements only with the express written permission of the Cooperative Education Teacher-Coordinator and following business practices for resignation. Students who fail to follow these procedures are subject to being dropped from Work-Based Learning. The student organization is an integral part of a student’s Career and Technical Education program. Therefore, all students are expected to participate in and actively support the Career and Technical Education student organization that relates to their career objective. When Work-Based Learning students honor their training mentors with a banquet, reception, etc., all students are expected to attend with their training mentors as their guests. Students are placed to train and are under the supervisions of the Cooperative Education Teacher-Coordinator, related study instructor, and training mentor where they are placed.

20 WBL: COOP Policy Students must abide by all school rules and regulations for other students and consider themselves under the jurisdiction of the school while at the WBL placement. Transportation to and from the WBL placement is the responsibility of the student/parent/guardian. Transportation problems do not justify absence from the WBL placement. Students will leave the campus immediately following the last scheduled class. If for any reason a student needs to remain on campus, permission must be obtained from the Cooperative Education Teacher Coordinator, School Administrator, or CTE Instructor. Other local additions

21 WBL: COOP Training Agreement
Student Expectations: Complies with the rules and regulations of the Work-Based Learning site. Observes the same regulations that apply to other employees. Adheres to all policies and regulations as set forth by school administration and the Cooperative Education Teacher-Coordinator. Is an active member of the student organization related to their career objective? Works an average of not less than 15 hours each week. Will not pursue additional part-time employment while enrolled in Work- Based Learning. Will not displace adult workers who can perform such work as assigned in the work-based experience. Attends an annual employer appreciation if required by the Cooperative Education Teacher-Coordinator or instructor of the career pathway course. Signatures Required by the following: Parent/Guardian, Student, and Employer

22 WBL: COOP Training Plan
The training Plan is an agreement between the student worker and the employer outlining the duties and responsibilities of the job. Students will have their employer to complete the form and the student will turn in to the assignment drop-box.

23 Time-Sheets All assignment submitted on the due-date
Expectation: To receive full credit for your assignment. ALL assignments must be Professional typed and submitted on the required forms located in Canvas Folder (No hand-written documents will be accepted) (Signature from both you and your employer must be on the Time-sheet. Completed form with the work schedule, the calculated total hours worked and hourly rate Staple both your time-sheet and pay-stub (copy) together before submitting to the drop box. All assignment submitted on the due-date

24 Time-Sheets All Work schedules, Timesheets, Pay stubs are to be turn in to the assignment drop-box located outside my classroom. There are 2 Time sheets posted in Canvas Apprenticeship –Paid Employment 2. Internship – Unpaid Employment (See slide 7 for detail definition). Please complete the time-sheet that applies to you.

27 Workplace Safety Test Review the PowerPoint on Workplace Safety
Upon completing the Workplace Safety review you will complete an exit Workplace Safety Test. You are required to score 100% Accuracy on this test. If not, you will be required to review the Workplace Safety PowerPoint again and re-take the test. Please complete the Workplace Safety Test. Print your test and turn in to the assignment box. Make sure you have scored 100% and your name is on the test before turning the test.

28 Performance Evaluation
Each student’s employer will provide the Instructor a Job performance evaluation once each grading period. The WBL: COOP evaluation grade will count as a Test grade and your semester exam grade (Term I and Term 2). No written Semester Exam will be administered to COOP Students. Students will report to their work-site for my class.

30 Late Penalty Late Penalty: Late Assignments will receive a 10 point deduction (the first 3 days). After the first 3 days, on the 4th day there will be 5 points deducted each day from your assignment final grade until the assignment has been turned in.  Please let’s be responsible young adults and turn in your assignments on Time!!

35 Course Syllabus  All students will receive a Course Syllabus outlining the expectation of the course. Most of this information was provided in the orientation video.  All students will be required to sign the documents related to the course during the WBL: Coop Orientation scheduled August 11-16, 2016.

36 WBL: COOP Orientation Test
Upon completing the WBL: Coop Orientation Video you will complete an exit slip-Orientation Test. You are required to score 100% Accuracy on this test. If not, you will be required to review the Orientation Video again and re-take the test. Please complete the Orientation Test. Print your test and turn in to the assignment box. Make sure you have scored 100% and your name is on the test before turning the test.
