Private Services Fee Schedule 2014 Advisory Services Fees
While there are indeed fees associated with the work, we would like to share that our approach to case management and other services is one intended to minimize expense, not increase it. Whenever possible, we provide case coordination, leveraging free or affordable services from community based services instead of creating new expense with CFPD . And while the fees may seem cumbersome when spelled out (our effort to remain transparent) when compared with other models, our overall fees remain at or below those of other nonprofits and certainly well below market rates. Advisory Services Fees Case Management $95.00/hour Assessment and Planning, including review of appropriate Services/Placement/Benefits and eligibility; As part of Court Process; Planning for Future Needs; Respite/Referral; Funds Management; Advocacy; Coordination of Services; Short-term or ongoing. A deposit of $250 will be collected at the initial meeting and applied to the first invoice. Trust Advisory $125.00/hour Advise Trustee on current policies related to maintaining SSI/Medicaid benefits; Recommendations on proper Distributions from Supplemental Needs Trust and Accounting back to Appropriate Agency; Public Benefits Analysis. (Minimum charge 0.5 hour) Conservator $95.00/hour Fund investment and management; Assessment and Planning; Coordination of Services; Court Reports and appearances. Witness Fee $125.00/hour As applicable to court appearances, including litigation. Administrative Support $70.00/hour Bookkeeping/accounting; bill-paying Shopping $30.00/hour Purchasing everyday items for client in person (Plus IRS Mileage Rate) or online (includes travel time). All time is billed in 6 minute increments. Postage and long distance calls at cost. The hourly fee will be reviewed annually and is subject to change. The Responsible Party will be notified in advance of any changes. Medicare Set-Aside Fees MSA Allocation Report $ CFPD staff review medical records and then work with the team (Attorneys, Medicare Beneficiary, family) to develop a Medicare Set Aside Allocation Report to consider Medicare’s future interest MSA Allocation Report with CMS Submission $ CFPD staff review medical records and then work with the team (Attorneys, Medicare Beneficiary, family) to develop a Medicare Set Aside Allocation Report to consider Medicare’s future interest. This plan also includes submission to CMS, which is currently required in all Workers’ Compensation cases. Administration of MSA Account Set up $ Once an amount for the MSA has been established, CFPD manages the funds that have been “set-aside” through a Set Aside Custodial Account or Set Aside Trust. Annual: For Funds $0.00 to $50,000 $1,000.00 $50,001 to $100,000 $1,250.00 $100,001 and Higher $1,500.00 Administration of MSA within Trust $70.00/hour If the Medicare Beneficiary has an SNT, the Medicare Set Aside is a sub-account of the Trust. CFPD will charge an hourly rate to manage the set-aside account. ***Responsible Party to initial services needed. Witness fee, administrative support and shopping may be billed for any client, as needed.***
