Most GP practices in Southend are smaller than the national average (about 7000) and in Shoeburyness 4 of the 5 practices are smaller than the national average Many GPs are approaching or are at retirement age NHS Property Services owns Shoebury Health Centre (and lease Shoebury House and Thorpedene Clinic) and the other 3 GP practices own their own buildings

Project Initiation Document (PID) approved by NHS Property Services and NHS England Funding released by NHS England to develop the business case Created new Shoeburyness Task and Finish Group CCG Governing Body approved Tenants Requirements Services to be located within the new premises being finalised Outline/indicative design of new premises underway

The Tenants Requirements set out the principles and vision for how the building will be used This enabled a Schedule of Accommodation to be developed which will help inform the design of the premises (see examples on next slide) This allowed us to assess the best site from the available options We want the building to be at the centre of the community The task and finish group will be involved in developing the design, as will local practices

The final options New building on Shoebury Health Centre site New building on Shoebury House site New building on Garrison site These options were assessed in August 2016 by a panel including local people to recommend the best option to the CCG and NHS England for approval. They considered criteria such as: location and accessibility size and sustainability deliverability impact on service delivery affordability

Local services currently provided in Shoeburyness include: Phlebotomy/blood taking (Thorpedene clinic) Speech and language therapy (Shoebury Health Centre) Stoma nursing services (Shoebury Health Centre) Therapy for you/ counselling (Shoebury Health Centre and some practices) Minor injury services (GP practices) Minor operations (gynae-related with Dr Sinha for her patients only, Dr Moss for his patients only) Diabetic eye screening (Shoebury Health Centre) Diabetes (Thorpedene clinic) Continence (Thorpedene clinic) Back clinic (Thorpedene clinic) Health Visitors Clinic (Thorpedene clinic) Children’s services developed through the ‘A Better Start’ programme

Changes in services over the last year Following your feedback we have increased phlebotomy provision at Thorpedene clinic to over 2000 appointments each week (also provided on a limited basis by some of the GP practices). There is more capacity if needed Dr Marasco offers appointments, access to medical records and repeat prescriptions online Drs, Kumar, Moss and Schembri are offering these online services as well as data coded records New dedicated GP service for residents of care homes to improve the quality of care and improve capacity in GP practices in late summer

Local GP Practice opening times GP Practice Opening times Dr Dhillon Monday, Tuesday, Thursday and Friday 8.30am to 6.30pm Wednesday 8.30am to 1.30pm Drs Kumar and Sinha Monday to Friday 8am to 6.30pm plus extended hours Dr Marasco Dr Moss Monday to Friday 8.30am to 7pm (closed 1pm to 2.30pm for lunch daily) plus extended hours Dr Schembri Monday to Friday 8am to 6.30pm GPs are responsible for their patients from 8am to 6.30pm, Monday to Friday. Extended hours are commissioned by NHS England.

We know that not all Shoeburyness residents are registered with Shoebury practices We know we need resilient and sustainable primary care services for the future The two Shoebury Health Centre practices will relocate to the new building and we are exploring options for a sixth practice for Shoeburyness This will help us provide accessible services to address the unique needs of the local population, provide capacity and resilience for the future and take advantage of the Big Lottery investment targeted at addressing needs of very young children and their families

We are part of the south and mid Essex Success Regime This is a national programme which is initially focusing on hospital services We have developed four distinct localities to support GP practices and deliver community services in Southend Services will be integrated across health and social care in each locality – with a new complex care service to launch on 1 October 2016

Southend East Locality More services to be moved from the hospital and delivered locally Consistent ‘offer’ of local services within each locality to ensure equity Better integration of services to provide patients with a single package of care Improved and coordinated services for patients with complex needs and their carers Primary care services aligned to the redesign of adult social care and community services - and delivered by multi-disciplinary teams Support the “prevention agenda” within communities Align, where possible, health services with; children services schools other health services such as mental health Working with the family as a whole and communities rather than just focusing on the individual
