KYC Documents Of The Proposer To Be Obtained
*Total annual contribution upto Rs 10,000. *Total annual contribution is more than Rs. 10,000 and less than Rs 1 lac. *Total annual contribution is Rs. 1 lac or above Recent Photograph Identification Proof Residential Proofs for both Mailing Address and Permanent Residential Address. Income Proof Legal Person Declaration [Not applicable for Individual case. Should be taken for non- individual cases only] Policy Serving Request Documentation required Address Change Proof of revised address Top-up Income proof document in case the Top-up premium is Rs.1 Lac or above Notes: When a customer applies for a fresh policy, address proof (Mailing and Permanent) should be obtained if the address mentioned on the new proposal from is different from the one mentioned in the existing policy Income proof should be obtained for a fresh policy if one year has elapsed from the last submission date in an existing policy. Assignment/Change in owner Photograph, Identity, address (Mailing and Permanent) and income proof of the assignee in case the Annual Contribution is Rs.1lac or above Claims Photograph, Identity and address proof document of the claimant Please note that all KYC documents submitted should be valid on the date of acceptance * Total Annual Contribution: Annual Premium paid by the client across all the policies held at BSLI.

Only latest ITR, audited accounts shall be accepted as valid income proofs. For eg : If an application is logged in after July 2010 (for individual case#), a valid ITR would be ITR filed for F.Y. March 2010 with A.Y Old ITRs upto 3 years may be accepted only if supported by any other additional income proof to verify the current source of funds. As per the current Compliance approved KYC list in place.

Format Of Certification Given By BSLI Compliance
Signature in the application should match with the Employer records The certificate should be current dated. The certificate should be obtained for every new policy. Dependent ID Card Learners driving license Hospital certificate (if the child is above five years of age ) Employment exchange card Provident Fund statement Court affidavit/Gram Panchayat certificate issued by other states (except Kerala) Invalid dates like 29th February 2009, 31st Nov, 31st June etc. Hajj Passport DOB is not clear in the given Age Proof If Nationality is Indian and client has submitted the Passport copy as an age proof then verify the place of issue and if the place of issue is other than India, take the age proof as "Not Acceptable." Also if as per passport nationality is other than Indian then take Age-proof as "Not acceptable.“ If WOP rider is opted for minor case, capture the owner DOB as per the business rule given for age proof data capture. Voter’s ID/Election Card. Ration Card. ESIS Card. Pension Card. Aadhar card without full DOB School/College Certificate (if child is school Going). Nursing Hospital Certificate/Discharge Card if minor is below 1 yrs. Birth Certificate up to 5 years of age. If WOP (waiver of premium) rider is opted for minor case, capture the owner DOB as per the business rule given for age proof data capture. If age proof is not submitted and previous policy details like previous policy number is given in the application form then answer ‘Yes’ for the question,” Is the Age Proof submitted? And accept it as standard age proof. If the difference in DOB between age proof and application form is up to 2 Yrs, capture DOB as per application form and accept it as ‘standard’. For immediate annuity and empower pension plan, all the standard and non-standard age proof to be considered as standard age proof. General Guidelines In addition please refer the general guideline which needs to be followed up as Business Rules while processing: Mail content (including the subject line) and the Attached document in the mail should be read thoroughly and verified before processing. Eg. If “Address proof not of Proposer” requirement has to be rejected, then the original Address proof requirement should be changed to ordered status and a new requirement “Address Proof not of Proposer” should be created with “Received but Not Acceptable” status and also giving proper rejection text. Rejection text should have the following content: Document type submitted, for which requirement it is submitted and the rejection reason. Eg. If ration card is submitted for address proof and the same cannot be accepted as address is mismatching, then the rejection text should be as follows: Submitted ration card for address proof is not acceptable as the address mismatches. Shortage requirement should never be changed to received status. If any requirement is received related to shortage, then CS Requirements Others should be created with received status and a valid text for the same should be entered. Eg. Letter received for mode change. For all Illustration following details need to be counter checked: Name of the Insured and Owner Age Gender Signature Fund options opted If any of these details mismatches, then the Illustration needs to be rejected. For letter and illustration signature should be verified before accepting it. For the entire supporting document like KYC, Age proof, etc. the customer attestation should be verified and signature to be tallied. For changes in the policy we will require fresh illustration along with the client consent letter specifying the changes on it. If there are any discrepancies on whether a documents can be accepted or not, then coordinate with BSLI compliance team through BSLI LCMP team.
