What is shared care? Getting involved in your dialysis care
Having the choice about what you want to do and don’t want to do Learning about your kidney disease and how it affects you Learning about your dialysis Improving your quality of life

How will it benefit you? It will make you feel more in control and less dependant on others It may reduce your waiting time in dialysis unit It will help you to understand what you can do to improve your health It will help you to understand what is happening when you are dialysing It will make you feel more confident

What shared care is not! It is not compulsory
It is not about cutting staff It is not about pushing for home haemodialysis unless that is what you are aiming for It is not about leaving you on your own. When you are in need, the staff will be available for you.

It is shared - not self - care

Evidence has shown that patients who are more involved in their chronic conditions have better health outcomes and enjoy a better quality of life.

Some of the benefits include fewer infections and better symptom control. Greater understanding of fluid and dietary restrictions and making healthier choices.
