The Value of Vision Offering vision benefits is an important way for employers to help their company’s overall health and wellness Approximately 75% of American adults use some form of vision correction.1 More than 80% of Americans feel it is important to have a vision care benefit.2 A slight miscorrection in eyesight can reduce productivity by 10% and work accuracy by nearly 40%. Computer eyestrain can reduce productivity between 10 and 50%.3 Eye exams can be used to spot early symptoms of diseases like diabetes, hypertension, multiple sclerosis, brain tumors, lupus, AIDS, osteoporosis, rheumatoid arthritis and Grave’s disease. Eligibility Guidelines, Contributions and Participation Mirror health plans options - Employee Elect: 50% contribution; 75% participation - BeneFits: 25% contribution; 60% participation Anniversary - Existing groups may add vision throughout the year; effective 1st of the following month. - Groups who previously had vision with Anthem Blue Cross Life and Health Insurance Company and dropped it, can again add vision coverage on their anniversary date. CalCOBRA and COBRA eligible Premium Only Plan (P.O.P) - Section 125 eligible An employee who declines coverage because he/she is covered by a spouse's employer’s group medical or vision plan is excluded in determining participation.

Member receives materials, in some cases within days or even hours. Member is reimbursed per their specific plan out-of-network allowances.
