Virginia Garcia serves over 50,000 people in Washington and Yamhill Counties. It is recognized as a best practice in delivering patient centered, culturally appropriate health services. Virginia Garcia is consistently achieving high health outcomes and low costs for total patient care. There is a high level of patient satisfaction and patient engagement. Patients have a sense that Virginia Garcia is a place that respects them, listens to them, and challenges them to help achieve their health goals. Virginia Garcia is also known as a wonderful place to work because staff have a deep sense of purpose for the work they do and feel supported to be creative and grow in learning. Key Results A key result is a quantitative statement that measures the achievement of a given objective. Quality Executive Sponsor: Laura Objective: Provide effective and evidence-based team based care Key Results: Meet 100% of required clinical quality metric targets Team Optimization—focused 1st on Care Coordination and Call Management Implementation of Visual Management tool on all care teams (contingent upon hiring of Clinical Team Leads) Call Management and EPIC in-basket documentation guidelines implemented Blueprint for successful spread of future Team Optimization changes across VG 100% of hospital admissions will be followed up within 7 days of discharge. We have found that we need to focus on Call Management and Care Coordination as 1st value streams, as, together, they provide the infrastructure for effective and efficient patient care. All of our care of patients happens through EPIC, so we need to standardize documentation and communication practices in EPIC as a foundation before optimizing other aspects of patient care. © 2008 VIRGINIA GARCIA MEMORIAL HEALTH CENTER

12 Care Team Whiteboard: Purpose and Expected Outcomes
The Whiteboard is a Visual Management Tool for organization and communication of the work of the Care Team. We anticipate implementation of the Care Team Whiteboard to achieve the following: Balance the workload across Care Team staff Ensure timely completion of care coordination work (e.g. In-Basket messages, forms, faxes, etc.), getting information to patients when they need it Facilitate communication between team members Improve staff satisfaction The Whiteboard also serves as a vital tool for guiding Clinical Team Leads through their daily standard work. Whiteboard and CTL is bringing a lot of this discussion to light. Previously, team members in their silos. Not much communication. Assumption that, when someone is out, there are other coverage models at the clinic level that are taking care of it. Team members not being engaged to help cover for their own team member. Efrain says that whiteboard helps him stay focused on his work and what his priorities should be.
