Philosophy The Program of All-Inclusive Care for the Elderly (PACE®) model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in the community whenever possible.

What does the Pace population look like
Vascular Disease Diabetes with Chronic Complications Congestive Heart Failure Major Depressive, Bipolar And Paranoid Disorders 47% Dementia Chronic Obstructive Pulmonary Disease 100% Need Nursing Home Level of Care 95% live in the community 5% live in nursing homes To qualify a person must be: 55 and older Live in a Pace service area Certified by the State to need nursing home level of care. Nursing Home Level of care is assessed through the DHS nurses and is based on a point system. Live in the community safely. There is no income eligibility requirements for participating in PACE. However most are Medicare and Medicaid duel eligible who have no out of pocket costs for the PACE benefits. If they are Medicaid only, then there is no additional costs. If they are Medicare only, then they pay for the Medicaid long term care premium and the Part D Prescription benefit. Which is a lump sum payment each month. Or they can private pay which is still cheaper than private paying for nursing home care. On Average Female 80 years old with HIGH care needs. Approximately 8 medical conditions ….. many of which are chronic conditions such as diabetes, dementia, coronary artery disease and cerebrovascular disease Although extremely frail, most PACE participants live alone in the community. When participants need nursing home care, they do not disenroll from PACE. The PACE program pays for nursing home care whenever necessary. Interdisciplinary team care follows them through all settings, including hospital stays and nursing home placements.
