If you are struggling to have a conversation about the Hope, Agency & Opportunity PROM, here are some ideas that might help you get started. They are all home grown ideas, things that have proved helpful to service users and staff here in Southern Health who are working towards recovery. There is a template you might like to try out, it builds a big picture of what each service user’s recovery might include. “The information you write will help us get to know you, and develop a care plan that is right for you.”
