3. Concept.
In personalized medicine, despite of the likeness in case, disease, clinical presentation management of the case differ from one individual to the other. This variation is due to different history; genotypic and phenotypic characteristics.
Also previous exposure to various conditions i.e., other diseases, drugs, foods that will determine how the individual will respond to the intervention strategies.
Thus better history taking increase the chances of success in the management of the case.

4. Concept..
The information obtained during clerk has to be communicated at various component of the health facility to ease the service provision;
Physician communicate with others in the setting, i.e., pharmacists, lab technicians and nurses through the history.
We are used to files share within the same setting but there is unaddressed challenge in sharing the history of the case outside the setting in case of referral.
Thus we need to share case across various health service points in a secure manner to enhance faster referral procedures while observing the privacy of the case. Thus a need for secure electronic sharing system rise, with consent from the client being observed.

5. Components and tools.
The major components in the system have two major parts
The clinical component and the biometric authentication system.
The clinical components includes the screening, diagnostic and therapeutic; management component, where the client is clerked, screened, diagnosed and case managed, the procedures followed forming the primary history of the client together with his or her personal particulars.
Secondly the primary history obtained are fed into the electronic system ready to be shared across the country’s health settings.
