Please include the University Purchasing Card Transation Log. If you are missing a receipt and cannot obtain one from the vendor, please submit a Missing Receipt Affidavit.

Bank statement attached
Cost of goods/services match total cost on bank statement and receipts correlate with purchases
Confirm no sales tax charged from NJ vendors (Rowan Tax Exempt Certification)
Tip:
When using complex image as full-bleed background add a transparency (70%-90%) fill layer to give contrast to text.
