Annual Open Enrollment
As New Hire, you have completed your waiting period and this is your once a year opportunity to add, waive or change existing coverage, elect or decline benefits for you or your dependents. Your effective date of benefits will begin the first of the month following your eligibility waiting period. Annually, we have an open enrollment for changes in May to have benefits effective July 1st. Next opportunity to make a change without a qualifying event will be May 2018 for an effective date of July 1, A qualifying event is defined as… Marriage Divorce/Legal Separation Loss of other coverage Birth/Adoption/Legal Guardianship Federal Facilitated Market Place Open Enrollment Period Death

Benefit Highlights Medical: Coverage remains with ASBAIT / Meritain and the BCBS Network. Employees have two medical plans to choose from: Copay Gold (Copay Platinum Plan is no longer available) Classic Gold District continues to pay 100% of the employee only on the Copay Gold plan Basic Life/AD&D: Renewing with current carrier Minnesota Life and at the current benefits. Dental: Will remain with Humana at the current benefits Voluntary Vision: Will continue with Ameritas at the current benefits and utilizing the VSP Network. Voluntary Life/AD&D: Renewing with current carrier Minnesota Life at higher Guarantee Issue amounts, increased dependent benefit. Voluntary Individual Benefits: Renewing with current carrier, AFLAC: No change to the benefit offerings of; Short Term Disability Accident Critical Illness Personal Sickness (Hospital Advantage Sickness Plan)

ASBAIT ASBAIT Medical = Blue Cross/ Blue Shield Prescriptions = Optum Douglas Meritain Name of the Trust that will provide medical insurance to Douglas Name of company that will process Douglas Unified’ s medical claims Name of the network Douglas will use for hospitals, physicians and prescriptions For medical services outside of AZ utilize the Aetna network.

Nurse On Call Nurse Support is a benefit free to you and your immediate family members. They can assist in locating the physicians, facilities, and programs that can make a difference in your health status. Nurse Support will help you understand what resources are available and how best to access those resources. 24 hours a day / 7 days a week (800)

Medical Definitions Copayment:
The fixed dollar amount you must pay directly to a physician at the time certain covered services, such as prescription drugs or office visits, are received. Deductible: The fixed dollar amount you must pay for covered services each Calendar year before the insurance company begins to pay benefits. Coinsurance: Sharing, between yourself and the insurance company, of the cost of covered services. Out-of-Pocket Maximum: The maximum dollar amount you have to pay under the terms of the insurance company’s contract in a Calendar year for covered services.

Life Insurance

Life Insurance - Minnesota Life
District Paid Life/AD&D Employee: Your annual salary not to exceed $150,000 You can also pay for the Basic Life benefit below (or enroll in the Voluntary Life) Spouse: $5,000 Children up to age 26: $1,000 ($100 if 14 days – 6 months) Voluntary/Optional Life Insurance Employee $10,000 increments to $500,000 maximum. Guarantee Issue of $250,000 when you are first eligible. If you are a current employee and want to enroll or increase your amount you will need to answer medical questions and Minnesota Life will approve or deny the coverage request. Current enrolled will be given $30,000 increase without EOI if below and up to GI. Spouse $5,000 increments to maximum of $250,000, not to exceed 100% of employee benefit. Guarantee issue of $50,000. For spouses of current employees who want to enroll or increase the amount of spouse coverage the spouse will need to answer medical questions and Minnesota Life will approve or deny the coverage request. Children up to age 26 $5,000, $10,000 or $15,000. Cannot exceed employee total basic and supplemental benefit level.
