2016 Maintenance of Certification (MOC) ENSW & SIM
To earn 2016 MOC Part IV credit, the submission deadline is November 28th 2016 All required materials must be submitted by that date Required materials: 1) Three quarterly submissions of clinical quality measures 2) Completion of the Attestation Form returned to 3) Payment of $75 per physician (there is no fee for faculty or volunteers of CU School of Medicine) More detailed information was included along with links to the Attestation Form in the PTO Weekly Newsletter If you have questions, please contact Allyson Gottsman

THANK YOU! To all those who participated in the second round of Collaborative Learning Sessions for cohort 1: Sept 9th & Sept 30th The Denver Metro Combined ENSW/SIM Collaborative Learning Session was attended by over 420 participants! The Western Slope SIM Collaborative Learning Session was attended by almost 100 participants! A huge thank you to Rocky Mountain Health Plans for their time and efforts related to the coordination of the September 30th session!

ENSW UPDATES: PRIME is now available to practices! More information can be found in the PTO Weekly Newsletter. PRIME Registry pulls data from EHRs and has expanded support to include CPC+, MACRA - MIPS, PQRS, and MU; Informational Webinars are also available by PRIME Registry staff Please encourage ENSW practices that can benefit from PRIME to fill out the Interest Form (found in the PTO Weekly Newsletter). Practices will be asked to upload physicians specific information (as outlined in the Provider List Template found in the PTO Weekly Newsletter). The PRIME Interest Form provides information needed to subsidize or pay for a PRIME license (if eligible). SIM Telehealth Updates
SIM Vendor Developing Statewide Telehealth Strategy Spark Policy Institute, the SIM vendor developing the statewide telehealth strategy will facilitate an interactive meeting/webinar on November 22, from 9:00 AM – Noon Findings on current state of ‘Telehealth in Colorado’ will be shared as they seek to develop a vision for expanding telehealth This will be a “stakeholder focus group” targeted toward providers and consumers; PTOs are a valuable bridge to our providers and important stakeholders An RSVP will be required; More details from the SIM Office will be available in the weeks to come

SIM 6 Month Practice Assessment Requirements
Review SIM Baseline Assessment Reports Being distributed to PTOs and practices this week. Practices and their matched PF’s and CHITA’s will receive Baseline Assessment Reports. These reports show a practice’s status and compare it to other practices in cohort 1. Review these with your practices at an upcoming meeting, and document IN YOUR FIELD NOTES areas of strength and opportunities for improvement. Successes and/or barriers to progress (as well as missed meetings and phone calls) should all be captured in your Field Notes. PLEASE NOTE: Expectations related to the Practice Improvement Plan (PIP) will be shared soon.

SIM CQM Reporting Deadline – November 1
Understanding and Reporting CQMs November 1 is the next CQM reporting deadline for SIM practices to submit third quarter CQMs Practices do not have to report on all of the core measures for their reporting group (adult vs. pediatric) for this deadline Practices should report on the same three core measures they chose for Q2 If they can report on additional measures they are encouraged to do so Example: if a practice reported on asthma, depression screening, and obesity for Q2, they should report on those same measures for Q3 and others (optionally) if they have the ability to Practices are not required to report on the additional core measures at this time
