2. A Brief History In Pa. it’s called Lifesharing
The first home was in 1983

4. Best Practices are concepts that the Pennsylvania State Lifesharing Coalition incorporates as the “quality” model for Lifesharing
These include:
Individualized matching: taking the time for people to get to know each other and pay the potential family for overnight visits
Provide choice. Trying to have variety of options for the people we support
Lifesharing Coordinator involvement with the family, not just a “monthly” meeting
Offering assistance with medical or behavioral appointments

5. Offering assistance with transportation
Developed a process for direct care staff for situations that have a high behavioral or medical need. (Supplemental Habilitation/Additional Intensive Supports- internal process)
When things are broken by the individual- prorated/depreciated reimbursement
Yearly family functions sponsored by us to provide support and inclusion with other families
We pay for family physicals, criminal checks, child lines and FBI checks
30 days of overnight substitute care- paid for by the agency
We provide all quality training and require insurance & heater cleaning verification
Starting stipend of $1750 (non-taxable)
Year end bonuses if we are in a surplus

6. Holding Host Families Responsible- paying the family ahead of time
To provide at least one (1) adult family member, who will have primary responsibility for caring for and providing good quality services to the Individual, who will satisfy all initial annual training requirements of the 6500 regulations. This includes a minimum of 24 hours per year of training. The Lifesharing Provider shall keep copies of all documents and materials relating to training source, contents, dates, length of training, copies of certificates received and persons attending. Furthermore, the Lifesharing Provider shall supply to KenCrest copies of any and all documents including certificates within five (5) business days of Lifesharing Provider’s receipt of same. Any and all training must be approved by KenCrest Lifesharing in advance. The training year is from May 1 to April 30 of any given year. The Lifesharing Provider shall obtain all required training as outlined in Exhibit "A", which is incorporated as part of this Agreement.

a) Payment to Lifesharing Provider may be withheld until required forms, documentation, logs, reports, and reviews are received by KenCrest as deemed necessary. Written Notice must be documented.

8. Holding Host Families Responsible- paying the family ahead of time
Because of the critical importance of a timely submission of the documentation as well as the necessity of the Lifesharing Provider obtaining all of the annual training hours, KenCrest reserves the right to withhold 100% of any of the Lifesharing Provider's monthly payments when the Lifesharing Provider is not in compliance with the requirement of this Agreement. KenCrest will notify the Lifesharing Provider of this withholding of funds via regular, certified mail and or . All withheld funds will be paid immediately after the Lifesharing Provider returns to full compliance. In the event that there is no compliance within 3 months, this contract will automatically terminate and the individual will be removed from the home.

9. Paying Host Homes after services are rendered
a) Lifesharing Independent Contractor shall have the right to provide Home Based Community Supports and Services and operate his/her business according to his/her best judgment in conformance with all applicable laws, policies and regulations and subject to KenCrest’s quality oversight. Based on Lifesharing Independent Contractor’s best judgment, subject to such laws, policies, regulations, and the ISP, Lifesharing Independent Contractor shall determine the schedule and manner of day-to-day activities, of the day-to-day management of the home, and of the performance of the Home Based Community Support Services.
b) Payment to Lifesharing Independent Contractor may be withheld until required forms, documentation, logs, reports, and reviews are received by KenCrest as deemed necessary.

10. Paying Host Homes after services are rendered
Additional requirements for payment
In order for a Lifesharing Contractor to receive payment for services rendered, they must also submit any necessary paperwork or training. This includes:
Prior month’s Monthly Fire Systems check (form provided by KenCrest, attachment 6, which meets the 55 PA Code, Chapter 6500 and Chapter 51 regulations)Prior month’s Fire drills (January and July, or additional ones as required) (form provided by KenCrest, attachment 7, which meets the 55 PA Code, Chapter 6500 and Chapter 51 regulations)
Prior month’s Monthly report on progress of individuals goals, calendar of activities, medical appointments, substitute care days and any other information as required by 55 PA Code, Chapter 51 regulations (form provided by KenCrest, attachment 8, which meets the 55 PA Code, Chapter 6500 and Chapter 51 regulations)
Prior month’s Monthly medications log (provided by pharmacy or KenCrest which meets the 55 PA Code, Chapter 6500 and Chapter 51 regulations) if required
CPR and First Aid training, which may not expire at anytime which meets the 55 PA Code, Chapter 6500 and Chapter 51 regulations
Annual heater cleaning, filter change or Fireplace/Woodstove cleaning, if applicable- current by February 29, 2015, which meets the 55 PA Code, Chapter 6500
24 hours of Training as required by 55 PA Code, Chapter 6500 and Chapter 51 regulations from May 1 through February 29 (see attachment 3)

11. Paying Host Homes after services are rendered
Annual copy of Homeowners and Auto insurance current by February 29, 2015, which meets the 55 PA Code, Chapter 6500 and Chapter 51 regulations
In the event, any of the above requirements are not provided; the invoice will not be paid until the requirement is fulfilled. Any questions or concerns about your required paperwork should be discussed with your Lifesharing Coordinator
Submission of invoice and paper requirements can be received via (to your LS Coordinator), fax (610) , US mail, in person or expedited mail.

12. Paying Host Homes after services are rendered
J. Premium Payment
If in the sole and exclusive discretion of KenCrest, Lifesharing Independent Contractor has satisfactorily fulfilled all of his/her obligations under this Agreement, including but not limited to, compliance with all statutory/regulatory requirements and has provided exemplary care to the individual(s), KenCrest may make an additional payment to Lifesharing Independent Contractor during the term of this Agreement.
