Employee Maximum- 10X salary or $500,000 (whichever is less). Insurability required over $150,000)
Dependent- $10,000 (children only)
Spouse Maximum- $250,000-Coverage on spouse cannot exceed 50% of employee coverage (Insurability required over $20,000)
