What is the Healthy Baby Program?

The goal of the Healthy Baby Program
To support expectant mothers throughout all phases of their pregnancy by ensuring: During pregnancy: Ensures women get prenatal care, understand maternity benefits, and have a resource for any complications. Personalized program focused on education, support and helping members have a healthy pregnancy and baby Identifies risk factors (e.g., smoking) and supports members in reducing health risks After delivery: Ensures that the baby gets newborn care; screen mom for post-partum depression; offers benefits and claims support. Ongoing, regular outreach throughout pregnancy and after delivery Improves health outcomes and reduces medical costs

Personal Health Advocates
This program builds on our Personal Health Advocate model, which addresses the range of issues that can surface around: Maternity care Care coordination for expectant mother Benefits assistance Fetal/neonatal care coordination Second opinions Chronic condition management Coordination of care for premature babies Post-pregnancy care coordination

What does the Healthy Baby Program include?

Reminders & Support Screening questionnaires specific to each trimester and postpartum. Telephone support call during each trimester (3) with reminders of each session. One post partum call with depression screening Total of 4 scheduled calls--plus unlimited additional advocacy support as needed.

Resources & Information
Tip sheets on such topics as: Breastfeeding Maternity Leave Pregnancy Q&A Understanding Labor Pelvic Strengthening Birth plan workbook and more When a member enrolls in this program, they receive a introduction letter, FAQ flyer and maternity book, which is currently “Mayo Clinic Guide to a Healthy Pregnancy”.

Registration, tracking & incentives
As claims data may not identify expectant mothers until late in their pregnancy, in order for Health Advocate to effectively manage this program, we would require that the members call to register, which will allow us to identify them at an early stage, as well as track their participation for incentive reporting purposes. Having an incentive strategy in place to drive enrollment would be important to a successful program launch. A typical incentive structure may look like: $250 for enrollment in Healthy Baby and $250 for completion
