Objectives: Improved sharing of Health Information to improve the citizen's experience of care and support business transformation Improved sharing of Health and Social Information to improve the citizen's experience of care and support business transformation Improvements in collaborative working between Health and Social Care IT Providers

Medical Interoperability Gateway (MIG) implemented across all U&EC providers eHealthscope and GPRCC project in phase 3 Nottinghamshire wide “Health and Care Portal” implementation underway and live at Nottm University Hospitals and Sherwood Forest Hospitals Social Care information now exchanged daily and on demand

17 Moving forward Vanguard foundation & lessons supporting system wide delivery Local Digital Roadmap (LDR) is the new five year plan Workstream 1: Information sharing Workstream 2: Infrastructure Workstream 3: Citizen access to information and care records Workstream 4: Digital maturity (How good are our systems and how well are we using them) Workstream 5: Assistive technology Nottingham looking to move at significant pace to support the STP (Technology Enabled Care is a high impact area) LDR is an opportunity to finance some of the integration work (at least up front and initial running costs)
