An employee appreciation program.

2 What is the Employee of the Mid Year (EMY) Program?
CDP’s EMY program was developed to recognize outstanding performance from our dedicated workforce. CDP employees who receive EMY have not only excelled at all levels of their work and performance with CDP, but have also gone above and beyond for given projects.

3 How to become nominated?
Outstanding and sustained performance of job description. Accomplishments outside of job description. Innovative or creative thought process leading to the enrichment of the company. Examples: Saving the company money, finding/researching useful and more efficient alternatives to practices, profitable company development suggestions. Improvement in CDP productivity and/or efficiency. Implementation, demonstration and/or encouragement of major safety improvement. Remarkable commitment/loyalty to CDP. Unbelievable team support: keeping fellow employees motivated, focused and enthusiastic. Exceptional care and success in client relationship building.

4 Who is eligible? How to nominate?
Any full-time, non-probationary CDP employee is eligible to be nominated for this award. No CDP employee is allowed to nominate himself/herself. How to nominate? To nominate any full-time, non-probationary employee, the EMY Form must be filled out and submitted to the Human Resources department, no later than the 20th day of June and/or December. All nominations are valid for the 6 months selected, only. There is no limit to how many employees can be nominated. Multiple employees can collaborate to nominate one individual. *Nominations should be for accomplishments in the current 6 month span.

6 Who Evaluates Nominations?
Nominations submitted/collected will be sent to employees (in “anonymous” excerpts) for voting. Each employee will vote for EMY and submit their vote to the HR/Office Manager. Once all votes have been collected/tallied, the EMY committee (a committee comprised of executives & managers) will select the EMY based on votes received. Administrators – HR/Office Manager and CEO are the Administrators/maintainers of this program. If you run into any issues, concerns, challenges, or have any questions please contact one of them for clarification.

7 When is the EMY Announced?
Mid Year will run: Mid Year 1 – January, February, March, April, May, June Mid Year 2 – July, August, September, October, November, December Announcements of the EMY will be no later than: Mid Year 1 – July 5th Mid Year 2 – January 5th If you have any questions, comments or concerns regarding the EMY Program, please direct them to Administration/HR.

8 EMY receives: 60 points in CDP’s Reward Points Program that can be used or saved toward program offered items. These points cannot be exchanged for cash value. Recognition in the CDP Newsletter. A letter of congratulations, highlighting their nomination reason. A company-wide announcement with their nomination reason. Their name on the CDP EMY Plaque, which will be displayed in the company office lobby.

15 When does the program start?
Due to program change happening early this year from EOQ to EMY, the first EMY will be awarded in July Nominations will be accepted for the months of April, May, June. Reminder - How to Nominate: Fill out the EMY Nomination Form and turn it into HR by June 20th. In this form you must know: Name of Nominee Nominee’s Department Basis of Nomination (Specific reason, project, idea or incident that prompted this nomination). Detailed Description of Nomination Nominator’s names are mandatory for HR Protocol, but will remain anonymous when evaluated by the evaluation committee. Employees cannot nominate themselves. Employees can collaborate to nominate one person. Any number of employees can be nominated.

17 What is CDP’s Reward Points Program?
CDP’s Reward Points Program was developed to provide employees with the opportunity to receive reward gifts beyond a single item applied to the employee community. CDP knows that one gift doesn’t fit all. CDP wants to provide options that would allow the employee to choose something that fits them personally. This program also allows employees to save points redeemable for larger rewards. All full-time, non-probationary CDP employees are automatically enrolled into the program.

18 How are points earned? You will earn points at two times per years of service at each of your employment anniversary dates {i.e. You will earn two points on your first employment anniversary date (2 Points x 1 year).} You will earn 60 points if selected as the recipient of CDP’s Employee of the Mid Year program.

19 Can points be forfeited?
Points do not expire, but the program does have a cap in place. Reward points will not be awarded to those who have a balance greater than 300 points. The employee is responsible for redeeming their points at or before the 300 point cap to avoid forfeiture. If your employment is terminated for any reason, your participation in the Reward Points Program will be terminated and the total balance of your accumulated points will be forfeited. Points will be forfeited if you voluntarily choose to cancel your participation in the Reward Points Program or the Reward Points Program is terminated. If your points are forfeited for any reason, we will not reinstate these points to your CDP Reward Points Balance.

20 How are points redeemable?
Points will be awarded and will be redeemed for rewards on a first earned, first redeemed basis. Points earned are available for redemption immediately. Once you accumulate the required number of points, you will be eligible to redeem them for your selected reward item. Once points are redeemed for a reward item, the employee may exercise sole control of the item (i.e. give to family member or friend). Contact HR to redeem points.

21 What can points be redeemed for?

22 Where can point balances be seen?
Reward Points Balances can be seen on individual paycheck stubs which are provided bi-weekly.
