ATE civil helper broke his leg while aligning a new welded flow line on sleepers. It unexpectedly rolled back from the sleeper and hit his leg. CAUSES : IP was checking the alignment by standing too close to the pipeline. No banksman available to give signal to the operator. LEARNINGS: Operator to check that the Plant is fully disengaged from any attachment before moving. Key Learning: Never operate the Plant without the presence signal of Banksman..
